利用RCA與FMEA提高醫療影像人為準確率

Using RCA and FMEA to Improve the Accuracy of Medical Imaging

摘要


RCA與FMEA已被廣泛運用在醫療產業上,用以確認醫療錯誤、不良事件發生的情形,並找出可能的原因,減少再次發生的機率。由醫院2008年之廢影像數統計結果發現,因人為因素所造成的廢影像約占總廢影像數的八成以上,故本研究只針對人為因素,利用RCA與FMEA進行分析並降低重照率,減少病患不必要的暴露次數。本研究利用影像計數功能統計2008年至2009年間,每月廢片之重照數。研究期間於2008年12月採用RCA與FMEA手法進行影像人為準確率之改善,並比較改善前(2008年1月至12月)及改善後(2009年1月至12月)之情形。在分析廢片產生之人為因素中,以病患移動或晃動、攝影部位錯誤及有異物或假影產生三者居多。透過制定SOP並加以宣導、增添媒體衛教影片及海報並擬訂放射師獎懲辦法等對策進行改善。結果顯示改善前(2008年1月至12月)之每月平均廢片率為5.80%;而改善後(2009年1月至12月)平均每個月之廢片率已降至3.19%。隨著醫療影像數位化,高重照率突顯出醫院品質管控的不當以及醫療資源的浪費,然而許多的人為因素是可以被避免的,藉由RCA及FMEA手法進行原因分析並提出對策改善,除了提升醫療影像照射之準確率外,亦能減少醫院成本及醫療品質。

We excluded the instrument factors, human factors only for the use of RCA and FMEA were analyzed according to the rate and reduce weight, reduce unnecessary exposure times. During the study period in December 2008 by RCA and FMEA conduct the re improvements according to rate and compare the improvement of the former and improved in the case. In the analysis of human waste generated in the film, to patient movement or shaking, photography site, wrong and a foreign body or artifacts created three majority. Before improve (January 2008 to December) rejection of the average monthly rate of 5.80%; after improved (January 2008 to December) rejection of the average monthly rate to 3.19%. With the digital medical images, the image the number of bad films, by counting the instrument functions at a glance. Highlighted the high rate of weight according to quality control inappropriate hospital and medical waste. Through the processes of improvement, human factors can be avoided to reduce health care costs and improve quality.
