一位13歲少女接受脊椎矯正手術之護理經驗

Nursing Care of a 13 Years Old Girl with Scoliosis Undergoing Spinal Diorthosis Surgery

摘要


本文旨在描述一位13歲少女,因重度脊柱側彎入院行脊椎矯正手術之護理經驗。護理期間為2011年4月1日至4月10日,筆者運用Gordon十一項健康功能型態為評估架構,藉與個案會談、溝通及身體評估後收集資料,結果發現個案有:急性疼痛、自我照顧功能缺失、營養狀況改變與少於身體所需與BMI未達標準體重過輕、身體心像紊亂健康問題。經由協助個案採圓滾木翻身減少傷口拉扯;穿背架支撐助行器;設計高蛋白增重飲食;重建對自我的價值觀,結果改善傷口疼痛;學習自我照顧技巧,可獨立完成活動;願意由鏡中觸碰自己的脊椎,克服身體心像紊亂的問題。

This report addresses nursing experience when treating a teenage girl, aged 13, who was suffering from major back pain due to severe scoliosis and who had undergone spinal surgery. By applying Gordon's 11 health functions model, the author collected information through communication and physical examination from 1st April to 10th April 2011. The results indicated that this patient had the problems with acute pain, deficits in self-care functioning and disturbance to her body image. Various measures were implemented including logrolling and the distraction method, reducing contact to the wound, relieving the acute pain and building self-care function. External bracing and a walker helped the patient to carry out exercise, reduce the pain and become independent. The provision of mental support allowed the patient to rebuild her self-esteem and self-value and thus overcome the disturbances to her body image. Together the above measures allowed the patient to regain a positive attitude and behavior allowing her to adjust to the life difference after surgery. The implementation of measures, such as logrolling and distraction method, reduced the damage to the wound, relieved acute pain and built self-care function. The external bracing and walker helped the patient to do exercise, reduce pain and become independent. The provision of mental support allowed the patient to rebuild self-esteem and self-value and thus overcome body image disturbance. This allowed the patient to have positive attitude and behavior to adjust the life difference after surgery.
