Successful integrated healthcare helps elevating healthcare quality and improving population health, and creates high-value service. Political factors, lacking trusts between different organizations, financial factors and different values among various health care providers are common stumbling blocks on the integration process of community care which lead to further difficulty in establishing high-value integrated services. The aim of this article is to explore the value of community care and the standard framework for integrated healthcare. Community healthcare providers should learn how to integrate community resources for the value-based practice in medical environment with limited resources.
