探討醫院員工健康體適能與下背痛之相關因素

Association between Physical Fitness and Low Back Pain among Hospital Staff

摘要


目的:探討醫院員工的體適能等級與下背痛之相關性。方法:採橫斷式研究,收案期間為2018年8月13日到23日,收集北部某區域醫院516位員工之體適能結果及北歐肌肉骨骼問卷資料。結果:研究對象平均年齡40.7±11.78歲、年資11.8年,曾發生下背痛佔47.09%,其中38.6%員工過去一年內經歷過下背痛,且多為痠痛性質(92.6%),持續1個月(34.9%)或1年以上(34.15%)。序位羅吉斯迴歸分析顯示,員工以已婚、體位肥胖者、曾有慢性疾病、每週平均工時超過40小時,以及體適能之仰臥半起(肌耐力)不是“非常好”等級,與罹患下背痛症狀持續一年以上的機率較高。結論:腹部肌耐力較好的醫院員工,罹患下背痛症狀的機會較低,透過肌耐力訓練、控制體重、每週平均工時40小時以內,為預防下背痛的關鍵,可提供職場健康促進計畫之實務參考。

Objectives: To explore the relationship between the level of physical fitness and low back pain among hospital staff. Methods: This cross-sectional study used a convenience sampling strategy to collect relevant data from a regional hospital in Taipei between August 13 and 23, 2018. We recruited 516 hospital staff and collected their data on physical fitness and NMQ. Results: The participants' mean age was 40.7±11.78 years, and their average time working at the hospital was 11.8 years. Almost half of the participants (47.09 %) had chronic low back pain, of whom 38.6 % had experienced pain in their lower back for the past one year, and felt sore (92.6 %) that persisted for one month (34.9 %) or over one year (34.15 %). Ordered logistic regression analysis showed that staff who married, with obese, with chronic illness, working over 40 hours per week, or had not-excellent results in curl-up test had a significantly higher risk of low back pain over one year. Conclusion: Hospital staff with a higher level of abdominal muscle endurance was less likely to have low back pain (over one year). Therefore, muscle endurance training, weight control, and working within 40 hours per week are the keys to prevent lower back pain. The findings may be referenced and applied in health promotion program practice.
