運用賦權概念協助一位脊髓損傷個案適應疾病之護理過程

本個案報告是描述運用賦權概念協助一位車禍導致頸椎損傷個案，適應疾病之護理過程。居家照護期間為97/12/9~98/3/10，筆者運用觀察、訪談、傾聽及身體評估等方式進行個案整體性的護理評估，歸納出個案有無望感、尿道感染、皮膚完整性受損等健康問題。筆者運用「賦權」概念在護理過程中，提供病患及照顧者適當之護理措施，引導個案反思脊髓損傷後的生理殘障，是自己無法改變而必須面對的事實，並尊重其決定是否參與照護計畫，目的是增進對生活的自主權、掌控感，以強化個案自我照顧的動機及能力，並減少無望感的產生，此外，筆者也加強連結社區資源，介紹脊髓損傷協會會員為其支持網路的一部份，重新建立希望找到生命的意義與價值，協助個案能適應疾病所造成的改變，重新調整腳步再出發，期許早日重返家庭及社會。

This case report described a nursing process of assisting a client with spinal cord injury due to traffic accident in adaptation of disease by using conception of empowerment. During the home care from Dec. 9 2008 to Mar. 10 2009, the author performed the entire nursing assessment including of observation, interview, listening and physical assessment, and conculded that the client had the problem of health inculding of hopelessness, urinary tract infection and impairment of intact skin. The author offered client and carer the adequate nursing care by using conception of empowerment in nursing process, and transferred their thinking into accepting the unchangable fact of physical impariment after spinal cord injury. The author also respected the client's decison to participating in their program of care. The author hoped it could increase the client's decision-making power, strengthen the motive and ability of self-care, and derease the feeling of hopelessness. On the other hand, the author also strengthened the link in resource of community including of introducing the member of the association of spinal cord injury as one part of supportive network, building up the meaning and worth of life again and assisting the client in adapting to change after disease, and hope the client could come back to family and community as soon as possible.
