摘要

本文主要描述一位60歲男性患者罹患口腔癌，接受口腔腫瘤切除及胸大肌皮瓣重建手術的照護經驗。護理期間自2009年4月21日至2009年5月12日，筆者於住院期間以觀察、會談、身體評估、直接護理等方式收集資料，採用Gorden十一項功能性健康型態為評估工具，確認個案有疼痛、呼吸道清除功能失效、言詞溝通障礙、身體心像改變等護理問題，病患因為外觀上的改變，從一開始面對暫時性氣切套管的不安到最後能正視及執行自我外觀的照護，針對個案擬定個別性護理計畫，協助克服疾病所帶來之壓力與衝擊，護理期間主動探視關懷個案、傾聽、陪伴及實際護理個案方式，改善個案生理上的不適，並教導非語言性溝通，重建自我身體心像及提升自我照護能力，促使個案早日重返社會。

It's describing with the nursing experience of a 60-year-old-male-patient suffered oral cancer who took the operation of oral tumor excision and pectoralis major flap reconstruction. The writer gathered those data by observing, conference, evaluation of body and nursing between 2009/MAR/21 and 2009/MAY/12 when the patient was hospitalized. Adopting Gorden's 11 functional health types as the tool of evaluation that makes sure those nursing problems with pain, the respiratory track cleans ineffectively, communication disturbance, and bodily image change.
Although the patient's appearance changed, it still makes the patient confront and overcome the uncomfortableness with temporary tracheostomy till nurses his appearance by himself. By this case we make a nursing plan that helping patients overcome the pressure of illness. During nursing period, we improve patients' physical discomfort and rebuild healthy mind by visiting, listening, accompanying, and nursing that makes them find their own life.
