Nursing Experience for Newly-diagnosed Rectal Cancer Patient Undergoing Colostomy

摘要

本文描述一位初診斷直腸癌病人，因腫瘤過大須接受腸造口術並接受後續輔助治療之護理經驗，護理期間為2011年10月25日至11月27日，藉由直接護理、會談、觀察、身體評估、病歷翻閱收集資料，以生理、心理、社會及靈性方面進行評估，分析歸納後確立個案有身體心像紊亂、無望感及知識增進準備度等健康問題。經由關懷、傾聽、陪伴及提供照護與個案建立治療性關係，在提供腸造口照護知識及實際執行相關措施、針對個案對於化學治療及放射線治療的疑問加以澄清後，個案漸能以正向態度面對疾病，正視自己身體外觀並執行自我照護，降低其無望、不安的情緒，進而配合接受後續的治療，期許本經驗能提供照護此類病人之參考。

This article described the nursing experience of a newly-diagnosed rectal cancer patient undergoing colostomy and subsequent adjuvant therapy as a result of an unresectable tumor. During the nursing period from October 25 to November 27, 2011, the author collected information by ways of nursing, direct interviews, observations, physical examinations, and medical record reviews to assess the patient's physical, psychological, social and spiritual aspects. The author observed that the patient suffers from health problems including disturbed body image, hopelessness, and readiness for enhanced knowledge. Through caring, listening, companionship, and nursing, the author built a therapeutic relationship with the patient. By providing the patient with sufficient colostomy care knowledge including practical measures of self-care and clarifying the patient's questions regarding chemotherapy and radiation therapy, the patient was gradually empowered to face the disease with a positive attitude. This article on nursing experience serves as a reference for caring similar patients.
