台灣糖尿病衛教師執行運動諮詢現況調查

A Survey on the Current State of Exercise Counseling by Diabetes Educators in Taiwan

摘要


本研究目的在探討國內糖尿病衛教師對病患進行有氧運動、伸展運動及肌力訓練等運動諮詢的情形,包括執行率、諮詢方式、內容、遭遇之障礙及影響其進行此三項諮詢的相關因素。研究的進行方式是以自填式結構性問卷來收集資料,對象為2004年衛生署公告的「糖尿病人保健推廣機構」共132家之糖尿病衛教師為對象。結果顯示有氧運動、伸展運動及肌力訓練三項運動諮詢的執行率分別為81.9%、58.5%及34.6%,給予病患上述三項活動建議量達到建議標準之比例則分別是72.3%、55.4%及26.9%。多數衛教師僅以口頭說明的方式進行諮詢,三項運動分別為87.3%, 81.6%及74.4%。有一半以上的糖尿病衛教師認為進行上述三項運動諮詢的主要障礙是缺乏運動諮詢衛教資料,此外對於有氧運動諮詢的次要障礙為沒有足夠的時間及病患不太可能改變或無明顯成效;對於伸展運動及肌力訓練的次要障礙為指導經驗不足,再其次是沒有足夠的時間。本研究結果可提供國內發展運動相關教材及糖尿病衛教師教育培訓的參考。

The purpose of this study was to investigate counseling practice concerning aerobic exercise, stretching exercise and strength training among diabetes educators in Taiwan, including the percentage of counseling provided, method and content of counseling, obstacles encountered, and factors that influenced counseling practice in relation to the above three items. We collected data using a self-administered structured questionnaire. The subjects were from 132 ”Health Promotion Institutions for diabetic patients” approved by the Department of Health in 2004. The results showed that the percentages of counseling practice provided on aerobic exercise, stretching exercise and strength training were 81.9%, 58.5% and 34.6% respectively. Of those educators who had counseled their patients, 72.3%, 55.4%, and 26.9% respectively reached the recommended standard of counseling, in terms of each item in the order listed above. Most exercise counseling was practiced only by oral instruction by 87.3%, 81.6% and 74.4% of the educators respectively, again in terms of each item in the order listed above. More than half of the educators considered that the major obstacle to exercise counseling for the above three items was lack of handy educational materials. The second biggest obstacles to aerobic exercise were lack of sufficient time, patient's unwillingness to change, and lack of obvious effects. The second biggest obstacle to stretching exercises and strength training was insufficient experience for instruction, while a minor obstacle was insufficient time. These results provide a unique reference for the development of exercise-related material and the future cultivation of diabetes educators.
