溝隙封劑的操作及發展現狀

Current Status of Fissure Sealants

摘要


溝隙封劑的使用目前被認為是預防窩溝齲蝕最有效的方法。雖然近年來有推出含氟的、或親水性的樹脂類封劑,希冀能提升預防齲齒的效果或改善疏水性材料使用上的限制,現今使用時的第一選擇仍以疏水性樹脂類封劑為主。在操作方面,酸蝕前的溝隙處理(包括牙釉質成形術、氣動研磨、鉺雅鉻雷射)及延長酸蝕時間的常規必要性,在實驗研究結果看來並不會對封填結果產生影響;而黏著劑的使用則被認為可以增加封劑的長期固持力。最後在安全考量方面,雖然有人提出使用樹脂類溝隙封劑會產生類女性荷爾蒙的物質而有潛在影響胚胎分化的危險,但目前研究證據皆否絕此點的影響性,並認為使用上是安全的。

Fissure sealants application is considered the most effective way of preventing pit and fissure caries. Nowadays, hydrophobic resin-based sealants are still considered to be the first choice of material although new resin-based sealants like fluoride-containing or hydrophilic type were inventing in these days hoping to improve caries prevention or using limitation of hydrophobic materials. Regarding the placement techniques for fissure sealants, the necessity of routine mechanical preparation of enamel before acid-etching including enamelplasty, air abrasion, and Er:YAG laser, and prolonging time for acid-etching seem not affect the results of sealing ability according to the laboratory studies. Finally in safety concern, although some raised up the potential risk of affecting human fertility from estrogen-like material deriving from resin-based sealants, so far the evidence denying this impact considered the placement is safe in human.
