This case report describes a 8-year-4-month old boy with upper anterior teeth malpositioned. This boy had extracted the mesioden previously. Clinical revealed the upper primary right central incisor did not exfoliate, and the upper permanent left central incisor erupted with rotation. After four months, the upper permanent right central incisor has erupted half, so we used removable orthodontic appliance with finger to correct upper permanent left central incisor, then expanded the maxillary and aligned upper anterior teeth by 2x4 fixed orthodontic appliance. After one year and four months, we achieved ideal outcome.
