摘要

當下顎乳臼齒過早脫落時，一般臨床上常使用下顎舌側牙弓維持器（lingual holding arch, LHA）做為空間維持器來保留繼生恆犬齒及恆小臼齒的萌發空間。除做為空間維持器之外，舌側牙弓維持器也可以用來增加矯正時的錨定作用（anchorage）。本病例報告包含兩位病例：第一位病人使用舌側牙弓維持器做為矯正錨定，局部矯治近心傾斜、萌發障礙的右下第二大臼齒。第二位病人在拔除骨性沾黏的右下第二乳臼齒後，以舌側牙弓維持器維持右下第一大臼齒的角度避免其近心傾斜占據萌發空間，在無使用其他矯正力量的情況下使繼生之右下第二小臼齒自然萌發至正常咬合高度。

Mandibular lingual holding arch (LHA) is a commonly used space maintainer for arch perimeter maintenance in case of premature loss of more than one deciduous molar or canine. In addition, LHA could reinforce anchorage in orthodontics when combined with the labial fixed edgewise appliance. These two case reports present different clinical applications of LHA. In the first case, LHA was used as an anchorage reinforcement in limited orthodontic treatment of mesial tilted and ectopic mandibular right second molar. In the second case, LHA was placed to prevent mesial tilting of mandibular right first molar after extraction of ankylosed deciduous mandibular right second molar. One year later, the lower right second premolar erupted spontaneously without any orthodontic intervention treatment.
