使用固定式指樣彈簧矯治上顎第一大臼齒異位性萌發：病例報告

第一大臼齒異位性萌發是混合齒列早期會出現的異常。其中有三分之二的案例在沒有外力介入下會自行「跳出」萌發至正常狀態，剩下三分之一則無法自行萌出，需要適當時機介入治療。若無適當治療，可能造成第二乳臼齒提早缺失、牙弓前後徑減少而空間不足及第二小臼齒阻生等。治療方式分兩類，牙縫插入和遠心傾斜或移動。前者包含黃銅線、彈力分離圈及分離彈簧，後者常見的有Halterman appliance、Humphrey appliance、指樣彈簧及頸部頭帶裝置。本案例是左上與右上第一大臼齒近心邊緣　卡在第二乳臼齒之不鏽鋼牙套遠心邊緣下，以固定式指樣彈簧助其順利萌發。

Ectopic eruption of permanent first molar is an abnormal situation that occurs in early mixed dentition. About 2/3 cases of first molar ectopic eruption can ＂jump out＂ spontaneously without ultra-forces. The other 1/3 cases can't fully erupt by themselves which need treatments at the proper time. Without appropriate treatment, it may cause premature loss of primary second molar, decrease of arch length that result in space loss, and impaction of permanent second premolar. There're two kind of treatment principles, interproximal wedging and distal tipping or movement. The former includes brass wire, elastomeric separator and separating springs. The later includes Halterman appliance, Humphrey appliance, finger springs and cervical-pull headgear. In this case, the marginal ridge of upper right and upper left first molar were impacted by the stainless steel crown of primary second molars and caused ectopic eruption. Fixed finger springs appliance was used to make them erupt to proper position as soon as possible.
