本專案為降低眼科住院病人轉送門診眼科檢查室之延遲率,2017年1月~3月,共有52人次需至門診眼科檢查室進行檢查,延遲送檢達43人次,延遲率達82.7%,造成醫師及病人無法準時檢查,影響後續門診及治療運作,引發寫此專案動機。經現況分析調查,送檢延遲率高為忘記開立醫囑、臨時接到通知、傳送輪椅分散各處、未統一使用電腦傳送系統、病人生理需求。進行改善策略:常規開立固定送檢時間醫囑、擬訂護理人員協助「眼科住院病人轉送門診眼科檢查室」之標準流程、統一運用電腦傳送下單系統、運用TRM手法增進雙方醫護團隊互動關係。2017年9月~10月送檢延遲率由82.7%降低至21.4%,提高準時送達率,醫師抱怨聲減少,連帶提升轉送服務滿意度。
This project was carried out to lower the delay rate of ophthalmic patient transfer to the ophthalmology examination room, there were a total of 52 people requiring the use of ophthalmology examination room from January to March, 2017, there was a total of 43 delays in examination transfer (delay rate 82.7%), resulting in both the doctors and patients unable to carry out the examination on time, affecting the subsequent medical operations, which therefore gave rise to the motive for carrying out this project. According to the analysis of the current situation, the high examination transfer delay rate was due to forgetting prescribing medical orders, receiving temporary notification, dispersion of the transfer wheelchair, unified use of computer to transfer medical order system was not used and the physiological needs of the patients. Implement improvement strategies: Prescribing medical order for fixed examination transfer time, formulate the Standard operating procedure (SOP) for nursing staff to assist with "Transferring ophthalmic patient to outpatient clinic examination room", unified use of computer to transfer medical order system and the use of TRM to improve the interaction relationship between the medical and nursing staff. The examination transfer delay rate decreased from 82.7% to 21.4% from September to November 2017. The efficacy of project promotion went well, as well as collectively promoted the satisfaction level of the transfer service.
