運用心智圖創新教學於氣切滑脫處置能力訓練之學習成效

The Effectiveness of Application Mind Maps Innovation Teaching Strategy Improving Nurses' Ability of Tracheostomy Tube Displacement

摘要


氣切滑脫會導致病人嚴重傷害甚至死亡,護理師具備氣切滑脫處置能力對病人安全至關重要。本單位51.92%護理師自覺氣切滑脫處置能力不足,28.9%沒有信心進行緊急處置,引發做此教學之動機。運用創新教學策略:以心智圖導引全腦思考聯想,輔以「流程圖」精熟處置步驟、「創意十字口訣」增強記憶、設計「氣切模擬人」熟悉解剖構造,並以「人形圖卡」及「設備圖卡」進行情境模擬演練。成效以「氣切滑脫處置自我能力評量問卷」評值其認知、情意與技能均顯著提升(p<.05),「氣切滑脫處置模擬訓練評量表」執行流程完整率達95.8%。結果顯示心智圖創新教學有效提升護理師對氣切滑脫處置能力及自信心,有助專業成長進而提升病人安全及照護品質。

Displacement of the tracheostomy tube is a nightmare in nursing care and can lead to insecure airway and patient death. Nurses' ability to deal with tracheostomy tube displacement is very importance for patient's safety. A previous questionnaire study in our surgical intensive unit show 51.92% nurses felt insufficient ability in dealing with tracheostomy tube displacement and 28.9 percent of all were not confident in emergent managent. Hence, we use multiple innovative teaching strategies including using a mind map to guide whole-brain thinking, using association, supplemented by "flowcharts" for processing steps, "mnemonic ten-word phrase" to enhance memory, design "the tracheostomy simulation model" to familiarize themselves with physiological anatomical structures, and conduct clinical situation simulation exercises with "human forms" and "equipment diagrams" are set up for practical training. We also use the "questionnaire for self-affecting ability to dealing with tracheostomy tube displacement" evaluate the effectiveness of their cognition, affection and skills have been significantly improved (p <.05), The integrity rate of the execution process of the "tracheostomy tube displacement simulation training scale" reached 95.8%. The implementation of the innovative teaching strategy of mind map that could improve effectively enhances learners' the tracheostomy tube displacement handling ability and self-confidence, which helps professional growth, competence to improve patient safety and quality of care.
