中醫藥配合放射線治療乳癌的病例報告

摘要

一45歲女性患者之乳癌病理報告為invasive ductal carcinoma, T2N0M0 (Stage II)，經部分乳房切除術及六次化學治療結束後，至本院接受放射線治療。患者希望減少放射性治療的副作用，如放射性皮膚炎及潰瘍、胸悶胸痛、胸脅苦滿、腋下痛等症狀及提昇機體的免疫力而就診。本病屬於中醫”乳岩、乳癰”的範疇，其病因是痰核結聚肝經而致病。於放射線治療前期，就中醫而言是熱毒傷肝肺經及脾胃，導致熱毒滯於胸中，法當疏肝解鬱、清熱化痰解毒、健脾燥濕。由本病例可以看出：乳癌在外科手術及化學治療後，以中醫藥協同放射線治療能夠成功的控制及治療乳癌，並明顯降低放射線治療的損傷性副作用，使患者完成整個放射線的療程，提高乳癌患者的生存率及生活品質。

A 45 year-old woman with breast cancer shows invasive ductal carcinoma, T2N0M0 (Stage Ⅱ) on her pathology report. She came to our hospital for radiotherapy after partial mastectomy and six times of chemotherapy. Patient would like to minimize side effects of radiotherapy, such as radioactive dermatitis and ulcers, chest distension and pain, distention in the hypochondriac region, auxiliary pain, and to maximize immunity. This disease belongs to ”breast rock” and ”breast carbuncle” that are caused by accumulated phlegm in liver meridian. In the pre-radiotherapy stage, it belongs to heat and toxins damaging liver, lung, spleen and stomach, and results in accumulated heat and toxins in chest. The treatment principle should focus on relieving stagnant qi of the liver, clearing heat, dissolving phlegm and detoxifying, strengthening spleen and eliminating dampness. It is clear that the combination of TCM and radiotherapy in breast cancer patients after surgery and chemotherapy can effectively control and treat breast cancer as well as prominently reduces damaging side effects of radiotherapy. Patients can successfully complete the whole treatment cycle with better survival rate and quality of life.
