摘要

本文目的為省思護理人員社區照護情境及角色功能，並呈現對社區個案持續性照護時，「社區精神衛生護理能力」實際應用的情形。護理過程從96年11月至97年8月為期9個月，分別以電訪、面談、家訪追蹤照護一位重覆住院精神分裂症病患。了解個案照護需求，包括自我價值與自信心之提升、持續接受藥物治療、疾病管理、連結社區資源建構支持系統等。根據台灣護理學會與中華民國精神衛生護理學會所發展的社區精神衛生護理實務能力指標，運用其中「治療性人際關係」、「建構自我」、「藥物管理」、「疾病管理」與「資源連結」等護理能力於病患的照護。結果顯示個案出院後病情穩定，生活作息規律，持續參與社區復健，也突破以往快速疾病復發再住院的困境。

The purpose of this paper was to reflect nursing situations and role functions in community, and demonstrate the application of the essential competence of community psychiatric mental health nursing in continuous care. The nursing care process was from November 2007 to August 2008. During the period of 9 months, the nurse provided cares by telephone, outpatient clinical services, or home visits to continuous following a patient with schizophrenia who had repeated hospitalizations. The nurse identified the patient's needs: promoting self-esteem and self-confident, pharmacological treatments, illness management, community resources and support systems etc. Based on development of Nurses Association, Republic of China, the nurse applied the essential competence including ”self construction”, ”medication management”, ”illness management”, and ”resources links”. The caring outcomes demonstrate the symptom stability, living regularity, attending rehabilitation program, family support, and decrease of re-hospitalization.
