An Exploration of Nurses' Experiences in the Care of Elder Patients with Psychiatric Disorders

摘要

研究背景：身心障礙者普遍會有提前老化的現象，且往往會有多元化的健康照顧需求，目前對於精神障礙者老化過程的照護經驗，研究著墨不足。研究目的：探討精神科護理人員的高齡精神病患的護理經驗，進而萃取研究對象的高齡照護經驗之主觀感受。研究方法：採質性的研究法，以立意取樣原則，針對符合收案條件的研究對象（N=12），透過半結構式指引，進行深度訪談，用錄音方式收集資料，再將逐字稿內容萃取出相關主題，並用質性分析軟體NVIVO 8加以歸納與分析類目。研究結果：研究對象對高齡者精神病患的照顧經驗感受呈現三個主題，分別為「對於照護病患問題的關注經驗」、「護理人員與病患或家屬互動之關注經驗」與「自己對執行照護高齡精神病患工作之角色反思」。結論與建議：本研究結果呈現精神衛生護理人員照護高齡思覺失調症患者的經驗與感受，有助於未來協助老年思覺失調症患者的護理服務時，提供老年護理照護的參考。建議於未來精神衛生護理人員的在職教育時加強培養周全性老人評估與老年護理的能力訓練。

Background: Disabled people with mental illness usually have the phenomena of premature ageing and thus have multidimensional health cares in need. Unfortunately, there have been insufficient research papers about exploring the ageing process and experiences in the care of patients with mental illness. Purpose: This study was to explore the nursing experience of taking care of elder patients with psychiatric disorders, and to extract the subjective perception of experiences shared from the psychiatric nurses in charge. Method: Qualitative techniques were adopted by using a purposive sampling method, following inclusion criteria for research subjective through semi-structured interview guild (N=12). Interviews with the patients were recorded and the themes were abridged from the text contents via the use of qualitative research software NVIVO 8 to further induce and analyze their categories. Results: Based on the analysis, three themes taken out of the experiences in caring elder patients with psychiatric disorder displayed as the following: (1) the experience of attentive problems with regard to taking care of patients, (2) the attentive experience of interaction between nurses and patients or their family, and (3) self-reflection on playing the caring roles in taking care of elder patients with psychiatric disorders. Conclusion: The results showed a true experience of inner perceptions for taking care of elderly patients with psychiatric disorders by psychiatric nursing staffs. These data are believed to be helpful to the nurses while taking care of older patients suffering from schizophrenia for reference. Our suggestion is that we should stress the ability of comprehensive geriatric assessment and implement geriatric nursing training programs as to continuing education for nursing staff.
