本專案旨在提升手術室護理人員對全膝關節置換術(Total Knee Replacement, TKR)認知與技能之學習成效,使刷手護理師能正確協助全膝關節置換術之手術過程,提升病人手術安全及照護品質。本專案中,採專家效度設計了「全膝關節置換術認知問卷」及「全膝關節置換術手術過程技能稽核表」作為認知與技能學習之調查工具。此外,對24位手術室護理人員進行認知與技能學習成效低原因分析,數據顯示出全部護理人員(100%)均表示手術器械多且複雜,91.6%表示缺乏TKR在職教育訓練,83.3%表示手術經驗不足。本專案針對此三項主要原因,擬定改善措施如下:(一)修訂「全膝關節置換術租賃器械圖檔」、(二)制定「全膝關節置換術工作手冊」、(三)製作「TKR器械擺放及組裝介紹」及「TKR手術過程」教學影片、(四)舉辦「TKR技能工作坊」等四項對策。本專案自2014年8月11日至2014年12月25日依計劃期、執行期及評值期三階段共為期4個半月;最後階段評值結果顯示全膝關節置換術認知正確率由改善前69.4%提升至89.2%,技能正確率由改善前66.3%提升至91.8%,皆達本專案目標值。
This project was designed to promote the knowledge and technology competence of the perioperative nurse for Total Knee Replacement (TKR). The experience scrubbing nurses can assist orthopedic physician in the TKR surgery to improve the safety and quality of caring patient. In this project, we developed two questionnaires which were a TKR knowledge questionnaire and a TKR technology checklist, using expert validity for collecting data of knowledge and technology competence. In the survey of poor knowledge and technology competence for 24 nurses, it showed that all nurse (100%) have the problem to identity of the complexity of surgical instruments, 91.6% lack of TKR curriculum, 83.3% insufficient TKR experience. Thus, the team members proposed the following four strategies for improvement: (1) re-edited TKR equipment diagram, (2) developed a TKR manual of operating procedure, (3) built up the video of TKR instruments setup and surgery process, and (4) conducted a TKR practice workshop. It took about 4 month 2 weeks to complete the plan, implementation and evaluation in year 2014. At the final evaluation, the data showed that the TKR knowledge accuracy rate increased from 69.4% to 89.2% and technology accuracy rate increased from 66.3% to 91.8%. The result reached our expected goals in this project.
