照顧一位罹患子宮平滑肌肉瘤病患之護理經驗

The Nursing Experience of Caring for a Patient with Leiomyosarcoma

摘要

罹患癌症對任何人而言都是莫大的衝擊，而住院及治療往往帶來許多焦慮、疼痛及生活型態改變的情形。本篇報告主要是探討一位罹患子宮平滑肌肉瘤病人面臨的心理衝擊及住院接受治療所呈現的護理問題。護理過程中筆者與個案及其家屬建立治療性信任的人際關係，於護理期間（93年8月6日至93年8月14日）運用Gordon十一項健康功能做系統性護理評估，並利用直接照顧、觀察、會談等技巧，評估出個案目前的護理問題有：急性疼痛／與手術傷口有關、焦慮／與健康狀態改變有關及睡眠型態紊亂／與生病住院有關。在護理期間筆者提供個案個別性、整體性的護理活動及相關資訊等措施，使個案勇敢面對此次住院壓力及接受疾病所產生的問題，希望藉此護理經驗能提供護理人員未來照顧類似病人時，除減輕術後身體不適外，亦能重視以同理心接納病人及家屬的心理反應和行為。

Cancer may cause a great impact on any person who suffers the disease, and it is usually associated with anxiety, painful experiences and changes in lifestyle for the patient during the course of hospitalization and treatment. In this case report the author intends to discuss the psychological impacts and the problems regarding nursing care during the hospitalization of a patient with leiomyosarcoma. In the process, the author was able to establish a relationship build on trust with the patient and her family. During the nursing period (from June 6 to August, 2005), the eleven Gordon's Functional Health Pattern were applied for systemic evaluation, and with the assistance of close observation, direct nursing interventions and communication skills the author was able to identified some nursing problems related to this case in particular including: acute pain/related to surgery, anxiety/ related to changes in life style, insomnia/related to hospitalization and the change of environment. In this critical period, the author was able to provide an individualize and al the same time integrate care of the patient, provide her with adequate information and measures to help her to affront the problems related to her hospitalization and treatment, and accept the great challenge of confronting her illness.
