一位初診斷膀胱癌行經尿道膀胱腫瘤切除術病人之護理經驗

Nursing a Patient with Newly Diagnosed Bladder Cancer

摘要


近年來隨著人們生活習慣的改變,癌症已連續22年位居國人十大死因之首,癌症不僅威脅病患的生命,同時還造成身體、心理及社會的衝擊。本文是探討一位中年病患面臨初診斷膀胱癌之調適過程,正面臨生活經濟重擔及照顧家庭成員的任務與責任,個案在得知自己病情後頓時間失去自信心,哀傷、淡漠隨之而來,更是面臨身、心、社會種種壓力及衝擊與疾病調適過程。由於個案處於中壯年期,自2007年2月23日至2007年3月5日,運用Gorden十一項健康功能型態評估作為護理評估工具,評估個案身、心、靈、社會、壓力來源及需求,確立個案有:排尿型態改變/與接受經尿道膀胱腫瘤切除術有關、個人因應能力失調/生命受威脅與角色職責受衝擊有關、知識缺失(特定的)/對於疾病瞭解及返家照護等主要健康問題。在照護過程中筆者以主動關懷、同理心、傾聽等支持技巧,並依各項護理問題提供個別性之護理措施,協助個案度過急性期生理及心理困擾,坦然面對疾病,並重獲自信心,為重返家庭及工作職場做準備。

Cancer has consistently ranked first among the top ten causes of death in Taiwan for 22 years. A diagnosis of cancer is immediately perceived as life-threatening, and the disease may have wide-ranging physical, psychological, and social impact. The author cared for a middle-aged man who had just been diagnosed with bladder cancer, managing him for 10 days before he underwent transurethral resection of the tumor. Gordon's 11-item functional health patterns were used to assess the patient. Issues he faced included the financial burden of his illness and concern about his family responsibilities. He lost confidence, faced grief over his condition, and appeared indifferent as he considered all the possible physical, psychological, and social pressures caused by the disease. He required help in understanding the illness and coping with its effects, such as changes in urinating habits. The author used active care, empathy, and listening to support the patient in this period of adjustment. The goal of appropriate individualized nursing care at this stage of the disease was to lay a good foundation for the patient's recovery after surgery. Recovery in this sense is not limited to physical healing but also involves restoration of self-confidence, relief from the acute psychological stress of the diagnosis, and a return to family life and work.
