摘要

本文旨在描述運用非侵入性輔助療法的音樂，協助一位孕婦緩解安胎過程所致的焦慮生心理反應之護理經驗，護理期間為2010年8月9日至2010年8月13日，運用Gordon十一項功能性健康型態評估為主軸，以觀察、會談、身體評估等方法進行收集、歸納及分析資料，確立個案主要護理問題為焦慮／對早產徵兆及安胎結果不確定，每天給予一次30分鐘之古典音樂，持續五天，並於音樂療法前、後以視覺類比量表測量個案的焦慮程度；同時在音樂療法前、後以醫療儀器進行生理測量。筆者除運用音樂療法緩解個案安胎焦慮不適所帶來的心理衝擊，更深入分析音樂對生理及心理指標之成效。期能經由此經驗分享提供給未來照護此類個案之護理人員作為參考，建議護理臨床推廣音樂療法，除一般的醫療處置外，能提供個案更適切及人性化之護理，促進婦女在安胎過程的身心舒適。

This article aims to describe the nursing experience of a nurse who used music therapy as a non-invasive intervention to reduce anxiety that a pregnant woman who faced physical and psychological reactions during the tocolysis period. The intervention was applied between August 9th and August 13th 2010. The author used Gordon's 11 functional health patterns to assess the patient's need. Data was collected by observation, face-to-face interview, and physical assessment. The major health problem was anxiety, which could be related to the symptoms of preterm labor and the uncertainty of tocolytic outcome. Music therapy was served as a nursing intervention. The patient listened to classical music with earphone about 30 minutes once a day, for 5 days. The questionnaire of Visual Analogue Scales was used measure to the patient's anxiety of the psychological impact. The changes of physical signs were measured on pretest and posttest design. In addition, the author used music therapy to relieve the patient's anxiety during hospitalized tocolysis, and applied an in-depth analysis of the effectiveness on physiopsychological index. This experience may serve as a reference for nursing staff in caring similar cases. In conclusion, the utilization of music therapy can be used to promote the comforts of women during the tocolysis and provides more appropriate and humanized nursing care.
