摘要

本文描述照護一位體重過重的精神分裂症病人之護理經驗，護理期間為2012年7月13日至2013年1月31日。筆者以整體性護理評估為指引，藉由觀察及會談收集資料，發現個案有：自我照顧能力缺失：沐浴及衛生；營養狀況改變：多於身體需要；知覺感受的變異：聽幻覺等護理問題。筆者與個案建立治療性人際關係，提供持續性的照顧，利用日間留院的復健醫療模式，訓練個案增進自我清潔的能力，並與案母、個案共同討論於日間留院期間，提供個別性的減重計畫，增加運動、記錄每天飲食及所需熱量等方式，讓個案體重從73.15 kg下降至71.15 kg；並提供專業知識及教導個案因應疾病症狀，幫助個案面對聽幻覺帶來的不適，藉以提升自我照顧能力及生活品質。

This paper was to describe a nursing experience on overweight schizophrenic patients. The period of care was given from July 31, 2012 to January 31, 2013. The Comprehensive Nursing Assessment guidelines were used for assessment. The data was collected by observation and conversation with the patient. There were major nursing problems that included self-care deficit: bathing and hygiene; altered nutrition: more than body requirements; sensory/perceptual alteration: auditory hallucinations. The authors established therapeutic relationship with the patient, provided continuous care, utilized day care hospital medical model of rehabilitation training case to enhance self-cleaning ability, and discussed individual weight loss plans, increase exercise, record daily diet and calories, etc. with the case and her mother during the case stayed in hospital. Such actions helped the case's body weight to decrease from 73.15 kg to 71.15 kg. Moreover, helped the case to deal with the discomfort from auditory hallucinations by providing the case professional knowledge and teaching to cope the symptoms of disease, and, so as to enhance self-care ability and quality of life.
