摘要

本文為照護一位思覺失調症個案服用抗精神病藥物導致體重過重之護理經驗，照護期間為2018年03月02日至2018年04月12日。筆者依精神科五大層面整體性評估收集資料，確立護理問題有：（一）營養不均衡：多於身體需要／服用抗精神病藥物、缺乏良好的飲食及運動習慣有關；（二）知覺感受變異：聽幻覺／與缺乏幻聽因應技巧有關；（三）長期性低自尊／與肥胖、持續性自我負面評價有關。因此筆者運用個別性飲食計畫、運動執行計畫，促使個案體重達成目標指標。同時協助個案區分幻聽與現實的差異，教導轉移幻聽干擾因應方法，並引導找出自我優點與長處來肯定自我價值，讓個案有信心面對未來的人生與規劃。

This article discusses an overweight patient treated with antipsychotic medication leading to overweight care. The care period was from March 02, 2018 to April 12, 2018. The author introduced five levels of care according to the psychiatric evaluation of information collected to establish care issues: (1) unbalanced nutrition: more than body needs while taking antipsychotics, leading to lack of good eating and exercise habits; (2) perceptual feeling variation in hearing hallucinations related to lack of auditory hallucination skills; and (3) long-term low self-esteem related to obesity and persistent self-negative evaluation. Accordingly, the author used individual diet and exercise execution plans to promote an ideal target weight to achieve the aforementioned indicators.
