降低手術病理檢體送檢不完整率

Reduction of the Incomplete Submission Rate of Surgical Pathology Specimens

摘要

手術病理檢體收集、運送完整才能正確即時提供病理資料作為治療診斷的依據，任一作業流程的不完整都會延遲病理報告，影響病人的醫療照護計畫。本專案實施前，手術病理檢體送檢不完整率達1.13%，分析主因為無跨院際運送規範、新進醫師及護理人員對作業流程不熟悉、交通車無放置專區、無檢體專屬櫃、運送箱及病人資料貼紙列印等設施。為降低送檢不完整率，新增跨團隊交班流程及簽收單、修訂新進人員教育訓練項目、規劃交通車檢體專區、新購置相關硬體設施。專案實施後，手術病理檢體送檢不完整率從1.13%降至0%，有效落實專案目的提升手術病理檢體送檢之完整並維護檢體之安全，參與運送團隊共同促進手術照護品質。

Pathological reports are important for clinical diagnosis and management. Surgical specimens have to be well collected and properly transported to the pathological department. Any operational error will lead to delay of pathological report and clinical diagnosis which has negative impact on medical plan decision and can impair patients' rights. Prior to this project, the incomplete surgical pathology submission rate reached 1.13% at our operation theater. The reasons accounted for this were: 1. Lack of inter-area delivery norms, 2. New staff, including doctors and nurses, were not familiar with the operating procedures, 3. No stationary area for the specimens in the transport vehicles, 4. No exclusive cabinet or transport boxes for the specimens, 5. No sticker printing facilities for patients' basic information. To improve these, we designed the new return document for handing over to the next working staff, educated new staff with training programs, established the stationary area in transport vehicles, and improved acquired facilities. After this project, the incomplete submission rate of surgical pathology specimens was reduced from 1.13% to 0%. The effective implementation of the complete surgical specimen submission not only maintains the security of specimens by the efforts of teamwork but also improves the quality of surgical care.
