最胸的癌症～一位乳癌轉移復發個案無望感照護經驗

摘要

乳癌復發的婦女，在家庭支持系統不足困境，面臨癌症轉移雙重打擊下，對生活及未來失去目標，導致個案產生無望感。本文探討一位55歲離婚女性診斷乳房惡性腫瘤併肺臟、骨頭及皮膚轉移之個案，在護理期間2017年12月22日至2018年2月05日，筆者經由觀察、會談、傾聽、身體評估、臨床照護等方式，以Gordon十一項功能性健康型態進行整體性評估，確立個案有「低效性呼吸型態」、「疼痛」、「無望感」，針對健康問題於照護過程中，藉由護理人員主動給予關心、傾聽、陪伴，協調跨部會醫療團隊持續介入，安排志工及教友協助，讓個案感受到醫療團隊的認真與用心，也漸漸開啟心門與醫療團隊互動，改善個案因疾病產生之生理不適及心理壓力，降低個案無望之情緒，真正提升病人生活與心靈的品質，這才是護理最終的目地。期許此次護理經驗可提供護理同仁日後照護此類病人之參考。

This article described 55-year-old divorced patient confronted with a breast cancer with multiple metastases suffered with hopelessness. The authors collected data through observation, physical assessments, direst nursing care, by the Gordon 11-functional health assessment from December 22th.2017 to February 5th,2018. The results revealed that the patient experienced three health problems, including 'ineffective breathing pattern, pain and hopelessness' Individual nursing program was then designed based on the patient's needs. Through the provision of caring, listening, and the help of volunteers and church members, the patient gradually accepted medical team. These methods reduced her uncertainty about the hopelessness from her breast cancer, improved the quality of patient's life and reachedthe ultimate goal of nursing. We provide this nursing experience in the hope that it could be a useful reference in related disease care.
