團體心理治療實務能力是精神衛生護理人員應具備的能力。然而護理人員願意主動接受團體治療實務能力訓練並接受臨床團體治療實務能力審核的比率普遍偏低,本病房審核通過率僅有17%。主要原因為:團體概念不足,計畫書書寫困難,缺乏團體訓練流程,沒有時間準備團體帶領及審核以及缺乏自信與動機。本專案目的在檢視護理人員接受團體治療實務能力訓練之成效。本團體治療實務能力訓練專案的策略包括:一、教育訓練及組成讀書會、二、輔導機制、三、建立團體訓練流程、四、提供排班的配套措施、五、營造帶領團體的正向氛圍、六、增強護理人員帶領團體的自信及學習動機。護理人員接受團體治療實務能力訓練後,通過團體能力審核率從17%提升到100%,足見團體治療實務能力訓練之顯著成效。
For psychiatric mental health nurses, one of the essential competencies is to administer and monitor therapeutic mental health interventions of group psychotherapy. However, the percentage was low in clinical psychiatric mental health nurses who were willing to participate in the training of group psychotherapy; whereas the average rate of nurses who completed and fulfilled the requirement of the competency appraisal in a hospital-based psychiatric unit in northern Taiwan was only 17%. The main reasons for this low rate included: insufficient knowledge of group psychotherapy, difficulty in group project writing, lack of group therapy training process, limited time to prepare for taking a group leader role, deficiency of self-confidence and learning motivation, and unwillingness to be appraised. The aim of this project was to verify the effects of a clinical training program of group psychotherapy for psychiatric mental health nurses' related competency. Strategies employed in this clinical training program of group psychotherapy were composed of: (1) forming study group and arranging educational training activities; (2) constructing counseling mechanisms; (3) establishing the training process of group psychotherapy; (4) providing supplementary measures when scheduling mental health nurses' shifts, (5) creating positive atmosphere of group leading; and (6) enhancing nurses' confidence and motivation during group leading sessions. The appraisal rate of clinical competence particularly for group psychotherapy has increased to 100% upon completion of the training program. Such remarkable effects of nurses' competence in group psychotherapy has brought the significance of establishing effective training program in a psychiatric unit.
