癌症病人在面臨疾病或治療過程經常受到多重症狀的困擾,因此近年來已有許多研究者進行症狀群集的相關研究。但是截至目前為止,症狀群集的概念定義仍無一致定論。本文運用Walker和Avant(2010)所提出的概念分析標準步驟,經文獻探討後,確立「症狀群集」的定義:(1)因疾病或治療的過程中,同時出現兩個或兩個以上身體或心理情緒不適的症狀;(2)群集中,症狀彼此關聯或會相互影響;(3)群集內症狀間的關聯性強於與其他群集內症狀的關聯性。本文提供實證性測量工具,並統合相關研究,提出對未來護理研究與臨床應用之建議,期能藉由增加護理人員對此概念的理解與掌握,進而正確應用此概念於護理評估與處置,以提升整體照護品質。
Cancer patients often experience multiple symptoms during their disease and treatment. The focus on symptom research in cancer patients have shifted from individual symptoms to clusters. However, the definition of symptom clusters is still unclear. The author used the Walker and Avant (2010) framework to guide an analysis of the concept of symptom clusters, and defined as: (1) two or more concurrent symptoms, (2) symptoms related to each other or had interrelation, and (3) symptoms within a cluster stronger than relationships among symptoms across different clusters. Through increased knowledge of symptom clusters, nurses will be able to provide personalized strategies to improve the quality of nursing care for cancer patients.
