檢體採集是臨床經常執行的重要項目,使用條碼科技於檢體採集作業可提高病人辨識,減少醫療疏失。檢視2016年因檢體採集過程錯誤導致病人治療延遲,經現況分析發現條碼追溯系統檢體採集正確率僅66.9%,原因分析為:採集檢體硬體設備不足、條碼機掃瞄技巧不純熟、資訊系統不穩定、護理人員未落實條碼追溯系統檢體採集流程。本專案目的為提升條碼追溯系統檢體採集正確率及降低退檢率,其改善方案為:製作教學影片及文宣、增置網路訊號、設計採血工作車、制定獎懲制度、規劃教育訓練、增強條碼敏感度。專案實行後,條碼追溯系統檢體採集正確率由66.9%提升93.5%,已達專案目標,並提高護理人員使用條碼追溯系統意願性。
Use of bar code technology in the collection of laboratory samples can improve patient identification and reduce error. Errors occurred during sample collection process may lead to delayed in medical interventions. In 2016, an analysis of the situation revealed that use of bar code traceability system sample collection rate was at 66.9%. Factors identified were insufficient hardware, unfamiliar with bar code technique, unstable information system, and nurse didn't use the system. The goals of this project were to increase bar code traceability system sample collection rate and to decrease sample rejection rate. Improvement actions included making a teaching video and protocol, improved the network signals, increased bar code sensitivity, designed a blood collection working mobile station, provided education and training, and developed a reward and penalty. Nurses' compliance to the use of the bar code system increased the accuracy of sample collection from 66.9% to 93.5%.
