某醫學中心洗手介入措施成效之探討

目的：比較洗手介入措施前後對醫療工作人員在各項洗手時機之洗手行為遵行率及正確率之改變情形。方法：以立意取樣選取南台灣某醫學中心所有醫療工作人員，透過行為觀察法，觀察醫療工作人員洗手行為遵從率及正確率共3186人次。以2008年1-4月作為前測資料，於5-12月介入各項洗手措施並收集後測資料。結果：在洗手介入措施後，在整體洗手遵從率由的68.4%上升至71.6%，洗手正確率由81.9%增加至90.5%且達統計顯著差異。醫療工作人員在「接觸緊鄰病人的環境後」的遵從率及正確率最低，而「接觸體液、分泌物、黏膜、受損皮膚、傷口敷料之後」的遵從率及正確率最高。護理人員在洗手行為之遵從率、正確率顯著高於醫師及其他醫事人員，而不同班別之洗手行為之遵從率、正確率則無差異。結論：洗手介入措施可增加醫療工作人員洗手遵從率及洗手正確率，亦可將此種介入措施應用在其他醫院、學校或社區上。

Objective: To compare the hand hygiene behavior correction rate, hand hygiene behavior compliance rate of hand hygiene intervention before and after.Methods: We collected about 3186 times of hand hygiene behavior by purposive sampling from a medical center in southern Taiwan. Add hand hygiene intervention from May 2008 to December, and compared with the outcome data from January 2008 to August.Results: The results showed that the overall hand hygiene behavior correction rate was elevated from 68.4% t071.6%, hand hygiene behavior compliance rate was significantly increased from 81.9% to 90.5%. The lowest hand hygiene behavior correction rate and compliance rate is ”after close contact with the patient's environment”, and the highest hand hygiene behavior correction rate and compliance rate is ”after contact with body fluids, secretions, mucous membranes, damaged skin, wound dressing” in medical staff. Nurses's correction rate and compliance rate of hand hygiene behavior was significantly higher than other medical staff. The hand hygiene behavior correction rate and compliance rate were not in different shift. Conclusions: Hand hygiene interventions can increase the medical staff hand hygiene compliance rate and the correction rate of hand hygiene. The interventions may also apply to other hospitals, schools or community.
