居家運動計畫介入對於頸痛患者頸椎關節活動度與疼痛之療效

The Effects of a Home-Exercise Therapy Programme on Cervical Range of Motion and Pain in Patients with Neck Pain

摘要


目的:現代社會的生活方式與工作型態,使用電腦與智慧型手機的時間上大幅增加,造成現代人頸部疼痛增加。本研究旨在探討居家運動計畫介入對本醫院復健科頸部疼痛病人的效果。方法:以30位頸部疼痛且有頸椎牽引的病人為對象,由專業物理治療師做頸部居家運動示範指導與衛教,病人到院接受第1次、第2次以及第12次治療,以捲尺測量頸椎關節活動度與視覺類比量表評估疼痛指數。統計方法以描述性統計與重複量數單因子變異數分析及事後比較了解頸椎關節活動度與疼痛程度的治療效果。結果:每一個方向的關節活動度與疼痛指數效果顯著性p<.001,達到預設值顯著水準且病人在第3次測量的評估有最明顯的改善,屈曲:5.38±0.53公分;向右側彎12.26±0.71公分;向左側彎:12.05±0.53公分;向右旋轉:11.39±0.39公分;向左旋轉:11.02±0.3公分;疼痛指數:2.59±1.1分。從事後比較發現,每個方向的關節活動度與疼痛指數在第2次測量結果優於第1次測量,第3次測量結果優於第1次與第2次測量,且平均數差異值均達顯著水準(p<.05)。結論:研究結果顯示居家運動介入對於接受頸椎牽引的頸痛病人在關節活動度與疼痛程度皆有明顯改善效果。

Objective: People spend much more time on smartphones and computers in the new lifestyles and working types of modern society. At the same time, the incidence rate of neck pain is also increased. The purpose of the study was to investigate the effects of a home-exercise therapy programme on patients with neck pain in a regional teaching hospital of southern Taiwan. Methods: 30 patients were recruited. The home exercises were demonstrated to the participants for patient education by physical therapists. Data were obtained by tape measurement and visual analog scale (VAS) with the 1^(st) ,2^(nd) and 12^(th) treatments. The data analyzed by descriptive statistics, One Way Repeated Measurement ANOVA and post hoc analysis with p <.05 significant level. Results: Results showed significant improvements in both cervical range of motion and pain severity. Flexion: 5.38±0.53cm; Right lateral flexion:12.26± 0.71cm; Left lateral flexion:12.05±0.53cm; Right rotation: 11.39±0.39cm; Left rotation: 11.02±0.3cm; pain scale: 2.59±1.1. Our post hoc analysis showed that the 3rd evaluation results had the greatest improvements. Conclusions: A home-exercise therapy programme was effective in increasing cervical range of motion and decreasing pain severity on patients who underwent cervical traction.
