Successful Treatment of Olfactory Neuroblastoma with IMRT: a Case Report
嗅神經母細胞瘤：強度調控放射治療經驗
摘要
嗅神經母細胞瘤(olfactory neuroblastoma; ONB)是一種來自於嗅黏膜的惡性腫瘤，也是在鼻腔中少見的腫瘤，傳統的治療方式是以手術進行切除，然而常伴隨著高復發率及高致死率。本案例是以強度調控式放射治療技術(Intensity Modulated Radiation Therapy; IMRT)治療ONB的報告。 對於ONB無法開刀的病患，IMRT合併每週cisplatin化學治療是另一種可以選擇的治療方式。因為IMRT可以給予腫瘤最大的放射線劑量，並保護正常的組織，可以將副作用減至最低。然而；不幸的是在治療的過程中因視神經受到過高輻射劑量照射而對病患視力造成損傷。因此雖然IMRT 的治療計畫可以減少正常組織接受的劑量，還是要注意所有可能引起的傷害以減輕正常器官的副作用。

Olfactory neuroblastoma (ONB) is a rare and aggressive tumor of the olfactory mucosa that has been traditionally treated with radical surgical excision. Nevertheless, these rare tumors are still associated with high rate of recurrence and mortality. Here in, I report a novel therapeutic approach using intensity modulated radiotherapy (IMRT). IMRT in conjunction with weekly cisplatin should be considered in patients with unresectable olfactory neuroblastoma since this technique can deliver an adequate tumoricidal dose. Careful IMRT treatment planning is necessary to avoid or minimize untoward side effect.
