摘要

目的：探討影響高齡醫學病房病人自覺需求與感受護理人員對其自覺需求瞭解程度的相關因素。方法：以台中地區某醫學中心之高齡醫學病房住院病人為研究對象，採會談問卷方式進行調查，共收案108位。結果：研究結果顯示高齡醫學病房住院病人自覺需求程度會因性別、住院次數及主要照顧者之不同而有顯著差異；高齡醫學病房住院病人感受護理人員對其自覺需求程度會因經濟主要來源之不同而有顯著差異。高齡醫學病房住院病人的自覺需求與感受護理人員對其自覺需求瞭解程度方面，其得分最高皆為環境設備。高齡病人自覺生理部分與感受護理人員瞭解程度之生理、心理、社經等需要方面呈現顯著正相關。高齡病人自覺環境需要部分與感受護理人員瞭解程度之心理、社經及環境設備等需要呈現顯著正相關。結論：本研究顯示高齡醫學病房住院病人的自覺需求與感受護理人員對其自覺需求瞭解程度，其需求得分最高皆為環境設備。研究結果可提供護理人員照護高齡長者的改進及未來教育訓練之參考，進而提升老人照護品質。

Objectives: To investigate the association between the factors affecting the self-perceived needs of geriatric inpatients and their perception of how well nurses understand their needs.Methods: 108 geriatric patients admitted to a medical center were studied. The data were collected by an investigator using the interview-questionnaire method.Results: The result identified gender, number of admission, and major care provider as the significant factors affecting the degree of self-perceived needs of geriatric patients. The major source of financial support of geriatric patients was found to influence significantly their perception of how well their needs were understood by the nursing staff. Hospital facility/environment ranked the highest in terms of both the self-perceived needs of geriatric patients and the degree of the nurses' understanding their needs. The physiological aspect of the self-perceived needs in geriatric patients was positively correlated with their perception of how well the nursing staff understands their physiological, psychological and socioeconomical needs. The environmental aspect of the self-perceived needs in geriatric patients was significantly correlated to their perception of nurses' understanding their psychological, socioeconomical and environmental needs.Conclusion: As indicated by the study results, the highest score in terms of both the studied geriatric inpatients' self-perceived needs and their perception of how well the nursing staff understands their needs goes to hospital facility/environment. The result can be applied to the care of geriatric patients by nursing staffs to improve the quality of elderly care in hospitals.
