從社區照顧關懷據點到巷弄長照站：挑戰與困境

摘要

本研究探討社區照顧關懷據點轉型提供初級長照服務將面臨的困境與挑戰，以嘉義縣社區照顧關懷據點為分析單位，並運用混合研究法的平行混合設計，以量化數據及質性訪談資料相互驗證及補充研究結果。研究發現，政府業管單位及專業輔導機構對於據點能量給予中等以上的評價，據點的行政和人力與其服務效能之間有顯著正向關連。據點承辦者是影響據點行政能量的決定性因素，尤其影響轉型提供初級長照服務的積極度與意願。據點的人力能量與其能否提供社會參與及健康促進活動有緊密關連。然而，在人力以志工為主，甚且年齡、來源涵蓋度與未來招募的前瞻性都較不被看好的據點現況下，勢必成為據點轉型提供初級長照服務的挑戰。

The objective of this study is to explore the challenges and dilemmas which the community care centers transform will encounter when providing primary LTC services. The community care centers in Chia-Yi County were the analysis unit of this study, and the parallel mixed designs of mixed-method were applied to verify the qualitative data and the quantitative data mutually. The findings indicated that the public sector manager and the NPO counseling agency of the community care centers all evaluated the competence above average that the community care centers transform to provide primary LTC services. The administration and manpower of the community care centers positively determined the competences that providing primary LTC services significantly. The undertakers of the community care centers determined the administrative competences, and especially the aggressiveness and the willingness that transforming to provide primary LTC services. The adequacy of manpower closely correlated with the activities of social participation and health promotion that the community care centers could provide. However, under the circumstance that the community care centers were facing the manpower problems that volunteer-based, aged and recruitment, it would be the challenge that if the community care centers transform to provide primary LTC services.
