Semiquantification of Tc-99m TRODAT-1 SPECT: Comparison Between DaTQUANT and MRI-Based ROI

Background: Dopamine transporter (DAT) brain single photon emission computed tomography (SPECT) is widely used to evaluate parkinsonian disorders. DaTQUANT (GE Healthcare) is a commercial software and has been validated for I-123 Ioflupane SPECT automatic semiquantification. However, its feasibility of DaTQUANT for Tc-99m TRODAT-1 SPECT semiquantification is unknown. To assess the feasibility, we evaluated the acceptable rate of automatic volume of interest (VOI) placement by DaTQUANT and compared semiquantitative data between DaTQUANT and the magnetic resonance imaging (MRI)-based region of interest (ROI) method in Tc-99m TRODAT-1 SPECT. Methods: This retrospective study (May 2021-July 2021) enrolled 45 patients who underwent Tc-99m TRODAT-1 brain SPECT and brain MRI within 6 months. DaTQUANT used a predefined VOI template to calculate the specific binding ratios (SBRs) of the striatum, caudate nucleus, and putamen. The MRI-based method used manually delineated striatal, caudate, putaminal, and occipital ROIs on MRI and transferred them to SPECT images to calculate the striatal, caudate, and putaminal SBRs. Results: The acceptable rate of automatic VOI by DaTQUANT was 97.8%. The striatal, caudate, and putaminal SBRs were 0.51 ± 0.30, 0.58 ± 0.32, and 0.47 ± 0.31 from DaTQUANT and 0.55 ± 0.31, 0.54 ± 0.32, and 0.55 ± 0.31 from the MRI-based method, respectively. Their concordance correlation coefficients were 0.919, 0.901, and 0.869, respectively. Systemic biases of striatal, caudate, and putaminal SBRs from DaTQUANT were -0.037, 0.038, and -0.077 compared with the MRI-based method. Conclusions: DaTQUANT automatic semiquantification appeared to be feasible in Tc-99m TRODAT-1 SPECT. The semiquantitative SBRs from DaTQUANT correlated well with those from the MRI-based method. However, biases between them are significant; thus, they cannot be used interchangeably.
