Increasing Nurses' Knowledge of Cancer Care and Their Skills in General Medical Unit

摘要

背景：護理師具備良好的腫瘤照護知能，可提升腫瘤病人照護品質及增加病人化療給藥安全，單位配合醫院政策將病房轉型收治以化學治療為主之腫瘤病人，完備護理師的教育訓練為首要任務，因此專案旨在提升護理師之腫瘤照護知能，專案執行時間為2015年4月1日至12月31日。經調查發現護理師腫瘤照護知能不足，原因為肝膽癌、胰臟炎及胃癌等腫瘤照護與化療標靶藥物種類變多，藥物作用與副作用之知識不足與無使用密閉管路輸注化療藥物之經驗。目的：護理師之腫瘤照護知識與技能由71.7%提升為90%。解決方案：透過在職教育、制訂化學與標靶藥物作用與副作用之簡版單張與修正化療給藥流程等措施。結果：護理師之腫瘤相關照護知識正確率由71.7%提升至94.6%，且完成全面使用密閉管路輸注化療藥物提升化療給藥安全，同時也增加護理師工作的成就感，以及增加病人對護理師的信任。結論：建議此模式可平行推廣至類似單位。

Background: Nurses with good competency in cancer care improves quality of care for cancer patients and increases safety of chemotherapy administration. More cancer patients are admitted to our unit for chemotherapy treatment. Therefore, it is urged to training nurses to enhance their competency in caring for cancer patients. The durations of this project are between 1 April and 31 December in 2015. The survey indicated nurse's knowledge deficits in cancer care because there are the lacks of the knowledge about liver, gallbladder, pancreatitis and gastric cancers, the different types of chemotherapy treatments and the effects and side effects. Moreover, they do not have the experiences in using infusion advice to provide chemotherapy administration. Aims: This project aimed to increase the correction rates of nurse's knowledge of cancer care and their skills from 71.7% to 90%. Interventions: In-service education, the leaflet with descriptions of effects and side effects of chemotherapy and targeted medication, and the process of chemotherapy administration were developed in this study. Results: The correction rates of nurse's knowledge of cancer care increased from 71.7% to 94.6. No safety issue of chemotherapy administration occurred during the project. This project also improved nurses' confidence about their competency in nursing care and enhanced patients' trust in nursing care. Conclusions: The interventions developed in this project can be applied to other medical units.
