某醫院護理人員疲勞與工作表現及環境之研究

摘要


目的: 探討醫院護理人員之疲勞、工作環境與工作表現之相關性,減少護理人員之流動率,增加留任意願及提升醫療品質。 方法: 以台灣中部某醫院之護理人員為研究對象,包括護理主管等共125人。本研究採用問卷進行研究,研究內容包括基本資料、個人疲勞自我檢測、護理工作表現測量(NPI)、嗜睡量表(ESS)、工作壓力調查表。 結果: 共回收92位有效問卷。女性占97.8%,年齡層以26至35歲最多占53.2%,每天睡眠時數6~7小時最多占34.7%;從嗜睡量表分析中得知,有38%的人經歷白天嗜睡;在工作壓力量表分析中,心理負荷的壓力源較多,以工作中同時處理許多事感到壓力最大﹔從工作環境問卷得知,有輪班者居多占59.7%,每週超時時數,超過6小時以上的高達17.4%;疲勞方面顯示,有64%呈現護理人員疲勞現況。從個人過勞輕微者占61%,中重度以上占39%;工作過勞輕微者占50%,工作過勞中重度者占50%;從相關分析得知,自我疲勞分數與NPI工作表現總分相關係數為-0.29(P<0.01);個人過勞分數與NPI總分相關係數為 -0.32(P<0.01),工作過勞分數與NPI總分之相關係數為-0.37(P<0.01) ,另教育程度與工作表現總分相關係數為-0.27(P<0.01)。 結論: 從疲勞與過勞量表與工作表現分析,多項呈統計顯著負相關,進一步支持疲勞在護理人員表現上所造成的影響;同時工作環境與護理工作表現是密切相關的,透過改變工作環境,它可能降低疲勞與減少醫療差錯的發生率。

Objective: To investigate the relationship among the fatigue of hospital nursing staff, environment and performance; to reduce the turnover of nursing staff, increase working performance and enhance the quality of care. Methods: The nursing staffs of a hospital in central Taiwan were sampled including the directors of nursing and other units, with a total of 125 nurses. The five kinds of questionnaires investigation, including basic information, self-test of personal fatigue, Nurse Performance Instrument(NPI), Epworth sleepiness scale (ESS) and work pressure were applied. Results: There were 92 valid samples collected. Female accounted for 97.8%; those who are aged from 26 to 35 accounted for up to 53.2%; those who daily sleep time from 6 to 7 hours accounted for up to 34.7%,In the analysis of Epworth sleep scale (ESS), there is a high proportion of 38% nurses who have experienced daytime sleepiness; in the analysis of work pressure scale, there are many sources of psychological pressure, in which the most stressful is to handle many things in the work at the same time. In the analysis of working environment scale, those shifts mostly accounted for 59.7%., those who weekly overtime hours, overtime than 6 hours for up to 17.4%. There were 64% nurses who experienced fatigue, those who individual overwork scales mild accounted for 61%, moderate to severe than 39%, and overwork scores severe accounted for 50%. In the correlation analysis showed that self-fatigue performance scores with NPI(Nurse Performance Instrument) total scores correlation coefficient was -0.29 (P <0.01),those who Individual fatigue scores with NPI total scores correlation coefficient was -0.32 (P <0.01), those who overwork scores with NPI total scores correlation coefficient was -0.37 (P <0.01). In addition, education with NPI scores correlation coefficient was -0.27 (P <0.01). Conclusion: In the analysis of fatigue , overwork and work performance, multinomial statistics are significantly negative correlation, which further support the impact of fatigue on the performance of the nursing staff; while working conditions and nursing performance are closely related , and it may reduce fatigue and the incidence of medical errors by changing the working conditions.
