口腔衛生師角色與權限

國內牙醫界目前對於口腔衛生師需求與存在的問題一直有著許多不同的意見與看法，害怕因此剝奪牙醫師的工作權，也擔心造成「密醫」的出現，對於口腔衛生師投入牙科醫療作業行列，顯得是既期待又怕受傷害。這幾年一直有相關人士在極力呼籲應儘早對口腔衛生師定位與規範，但截至目前為止卻仍是只聞樓梯聲、不見人下來的情況，因此引發本研究動機。本研究採用德菲法，旨在探討研究口腔衛生師的角色與權限，將各項議題依照研究結果的重要性作優先排序。本研究共進行兩次問卷調查，在第二次問卷時，專家們即已產生共識性。第一次問卷寄發產界110位專家，回收 95份，回收率為86.36%。第二次問卷寄發產、官、學、業、民眾各界225位專家，回收172份，回收率為77.77%。專家認為口腔衛生師的角色與權限應先通過立法後，訂定出口腔衛生師可行之工作項目與範圍，以達至口腔健康促進，並提升牙科醫療服務的深度和廣度。專家認為在「贊成職稱」上以口腔衛生師和口腔衛教師最為恰當；在「職務角色」上以「口腔衛教教育者」和「專業口腔預防照護者」最適；在「功能提升」上以「序號1」至「序號7」最適；在「具備技能」上以「序號1」至「序號9」最適；在「待遇標準」上以「同醫事技術人員」最適；在「綜合討論」上以「序號1」至「序號3」最適。研究結果，口腔預防保健暨各方牙科發展等各項效益均需要由口腔衛生師與牙醫師之間相輔相成、分工合作。口腔衛生師協助與管理可有效降低齲齒率，符合世界衛生組織標準，提升牙科醫療品質暨提升牙科實質上收益。國內健保給付，僅醫師、牙醫師、中醫師可合法分享健保給付，無需憂慮合法化有分享健保給付之虞。口腔衛生師職務層級屬醫事技術人員，薪資可比照醫事技術人員再依各能力做額外加給。口腔衛生師存在性已越趨顯著需要及勢在必行。依照各項議題之重要性順序擬定口腔衛生師角色與權限，在此提供給各界作參考使用。

At present the domestic Oral Hygienist for oral needs and problems has always been a lot of different opinions and views of dentists fear of thereby depriving the right to work, but also worry the “unlicensed doctors”. We expect the Oral Hygienist for oral health care operations ranks. But also afraid of something wrong. Too many years past, nobody care and face up to Oral hygienist who must joins necessarily to Dentist system. I will discovery and focus the position of Oral Hygienist to right way and suitable rules. This study used the Delphi method to investigate the oral health research about the role and authority, in according to the result of research to decide priority. This study have twice questionnaires, in the second questionnaire, already have the consensus of our experts. The first 110 questionnaires sent to experts in estate sector, 95 recovery rate was 86.36%. the second questionnaire sent to personnel of industry, government official, scholar, public sectors 225 experts, 172 response was 77.77%. The oral health experts believe that the oral hygienist of role and authority should be passed by the Legislative Yuan. We hope the oral hygienist join the dentist system will improve oral health and promoted best services for whole people in future. Almost experts agree the oral hygienist title use the Chinese name Oral Health Education better than Dental Hygienist. In the role part, they vote good call in Chinese is Oral Hygiene Educators and professional Oral prevent health care. In"advance function"they choose no1 to no7. In"ability for work"they choose no1 to no9. In"salary", they choose the same of Practice of medicine specialists. In"general discussion part", they vote no1 to no3. The results of this study, oral health care can help and share the dentist workload. Division to assist with the management of oral hygiene can reduce the caries rate and meet standard of World Health Organization. Also to improve the quality of dental care. National health insurance payments, only valid to practitioner physicians, dentists and traditional Chinese medical. Don’t worries others shares benefits. Oral health division has become increasingly obvious and imperative necessary for a long time. Now, we know the oral hygienist is very important to join to dentist system in Taiwan. It is necessary to quickly to passed by the Legislative Yuan and clear the oral hygienist role and authority for Taiwan dentist system development.
