摘要 隨著資訊科技的發展,醫療院所紛紛導入資訊化流程來提升醫療服務品質及效率,資訊化流程包括在院內及院所間推行醫療資訊系統,然而跨資訊系統的整合必須解決資料交換的問題。在偏遠地區巡迴醫療方面,雖然政府單位已進行導入階段的計劃,但是在醫師看診設備及病歷方面並無太大的進展,仍然使用舊有的設備及紙本病歷。為改善在巡迴醫療時,查詢不同醫院的病歷所進行資料轉換的流程,本研究遵照醫療資訊協定第七層(Health Level seven, HL7)定義資料格式,設計與建置巡迴醫療病歷系統雛型,以XML(eXtensible Markup Language)做為電子資料交換編譯格式,來整合跨醫療資訊系統的資料交換。為克服偏遠地區資訊設備不足,考慮不增添現有資訊設備下與合作醫院於區域網路內進行巡迴醫療資料交換,採用簡單物件存取協定(Simple Object Access Protocol, SOAP)技術負載所欲傳送之方法及參數。醫師於巡迴醫療服務時,執行佈署於行動通訊設備或手提式電腦上之相關程式並輸入參數,經由系統傳送至巡迴醫療團所屬之醫院或合作醫院,啟動應用程式進行病患病歷之查詢及儲存。本研究透過建置此巡迴醫療資訊系統以彌補現行紙本病歷之不足,增加病歷之流動性及使用性,達成跨醫院醫療資訊共享的目標,進而提升台灣巡迴醫療服務之效能。 關鍵字:巡迴醫療、醫療資訊系統、HL7、XML、SOAP
Abstract With advances of information technology, the government in Taiwan made many efforts to improve and integrate medical information systems in major medical institutes to provide better medical services. However, up to date the medical information requirement of mobile medical service for residents in aboriginal areas and islets was not well considered and fulfilled. The medical data formats may be different in various hospital databases and it becomes a problem for medical data integration in mobile medical services. This thesis proposed the architecture of a health information system to integrate electronic patient records for the mobile medical service. An XML document conforming to HL7 as an exchanging standard was proposed for medical records integration among hospitals. Information equipment is not sufficient in remote areas and there are many limitations for medical doctors to accessing the hospital information system. SOAP was applied to carry exchanged methods and medical data to fulfill the medical record requirement of mobile medical teams. SOAP application programs are disposed on application server in the medical institutes, and they can be executed remotely by using mobile devices and notebook computers. Therefore, doctors in the remote area can query or key in medical records of patients with the aid of mobile devices or notebook computers. This initial effort provides the primary solution for the information requirement of mobile medical services, and hopefully it may lead more research interests in this area.
