我國急診醫師生涯發展及其相關因素研究

A Study of Career Development and Related Factors of Emergency Physicians in Taiwan

摘要


基於急診病患日益增多與疾病複雜性高雙重因素的考量下,強化急診醫療體系確為當前衛生主管機關重要且迫切的任務。急診醫師身為醫療市場最主要的供給者,其對醫療工作的投入程度將直接或間接的影響急診醫療醫療的品質。為謀求醫療品質與醫師生活保障雙重前提下,實有必要針對急診醫師的生涯發展及其相關因素進行探討。 本研究的目的旨在1.瞭解急診醫師人力發展現況2.瞭解急診醫師的工作現況3.探討急診醫師的職業生涯發展的影響因素4.瞭解急診醫師的職業生涯發展情況5.探討急診醫師的職業生涯發展的輔助措施。 本研究以全國現職急診醫師為研究對象,並以立意取樣選取中華民國急診醫學會之醫師個人會員作為研究樣本進行問卷調查,以瞭解急診醫師之個人特質、工作現況與職業生涯發展情況與相關因素。發出514份問卷,回收有效問卷共計121份,回收率為23.54%。依據研究結果,本研究獲致下列結論: 1. 目前醫師人力總供給呈現穩定成長趨勢,而急診醫師隨著衛生主管機 關的政策走向與立法規範下,正逐步朝向專業化發展。 2. 急診醫師的工作現況調查顯示急診醫師從事急診工作的年資平均約當 於其從事醫療工作年資的一半,服務機構主要為私立醫學中心與私立 區域醫院,工作職責則以內科系與全科急診醫療為主,且多數急診醫 師並未擔任教職。 3. 急診醫師選擇申請急診專科醫師資格的主要原因依序為「有意從事急 診工作」、「基於個人興趣」與「符合申請的條件」。 4. 急診醫師的職業生涯發展情況調查顯示急診醫師多數有轉換科別或機 構的經驗,而且目前以處於早期至中期的生涯階段者居多;急診醫師 預期離開急診工作時間平均為47歲,在離開急診工作後的職業規劃仍 以從事臨床醫療工作為主,且急診醫師在離開後重返急診工作的意願 並不高。 5. 急診醫師認為對其職業生涯發展最具正面影響為「工作挑戰性」因素 ,最具負面影響則為「醫療糾紛發生率」因素,而且急診醫師對於急 診醫師職業生涯發展影響因素的知覺,會受急診醫師的基本資料、工 作現況及職業生涯發展情況的不同而有所不同。 6. 急診醫師認同的職業生涯發展的輔助措施主要在於對急診醫師個人權 益與安全的維護,以及平衡急診工作本身的投資報酬。

The purpose of this study is to understand the supplement of emergency physicians, the occupational characters of emergency physicians, the career plans of emergency physicians, the related factors of career development of emergency physicians, and the assistant policies of career development of emergency physicians. The sample of this study includes all of the individual members of Emergency Service Association The instrument of this study is a questionnaire includes personal characters, job characters and career plans of emergency physicians. Data was analyzes by the methods of descriptive and inferential statistics, including one-way ANOVA, t-test, and correlation analysis. The results of this study can be drawn as following: 1.The supplement of physicians in Taiwan is increasing on a steady trend, and the emergency physicians become a professional career by legal regulation. 2.The average past emergency serving years of emergency physicians is just a half of their average past medical serving years; most of their serving institutions are private medical center and private area hospital; most of them charge internal and non-specialty medical service, and does not serve as a teacher. 3.The main reasons of emergency physicians to apply for the certificate of emergency specialty physician are 'want to engage in emergency service', 'base on personal interest', and 'fit the application requirement'. 4.Most of the emergency physicians have the experience of career turnover, and their stage of career is between the early stage to the middle stage, the average quit age is 47 years-old, and most of them don't want to come back to the emergency medical system. 5.There are significant differences among emergency physicians ,grouping by their personal characters, job characters, and career plans, in perception of the related factors of career development of emergency physicians 6. The assistant policies of career development of emergency physicians should be focused on keeping the rights and safeties of emergency physicians, and balancing the costs and rewards of emergency service.
