病歷是病人就醫過程中所有的診療記錄,亦是健保支付、司法機關鑑定、以及教學研究的重要依據。傳統紙張病歷有著辨識與保存不易、及傳送緩慢等問題,隨著環境變遷與科技進步下所發展出的電子病歷,可以有效解決這些問題。相關研究目前仍缺乏瞭解醫師對於電子病歷系統滿意之影響因素;因此,本研究以醫師個人觀點,探討影響電子病歷系統使用滿意之因素。 本研究以期望確認理論為基礎,透過文獻探討並實際訪談醫師意見結果來建構研究模式。本研究並提出「提昇醫療服務品質之期望」的概念,參考SERVQUL量表來發展「提昇醫療服務品質之期望」量表。然後,以問卷調查法進行資料收集,共發出450份問卷,有效問卷338份。資料分析結果顯示,影響醫師使用的滿意度因素包含認知有用性、確認、提昇醫療服務品質之期望、及認知相容性;影響醫師持續使用意願因素包含認知有用性、認知風險、提昇醫療服務品質之期望、主觀規範、滿意度。最後,本研究提出可提供學術研究、政府機構、醫療院所、資訊人員、及相關資訊廠商等參考的建議。
Comparing with the traditional paper-based medical record, the electronic medical record (EMR) is easier to identify and store, and faster to transmit as well. However, we still know little about what factors influencing physician’s satisfaction on EMR system use. This study aims to investigate influencing factors of physician’s satisfaction on EMR system use. The research model was conducted based upon the Expectation-Confirmation Model. The measurement of “expectation on medical service quality improvement” was then developed.. Finally, a survey on Taiwanese physicians was conducted to empirically test the model. We distributed 450 questionnaires and 338 effective responses were collected. The analytical results indicated that the factors significantly influencing physician’s satisfaction including perceived usefulness, confirmation, expectation on medical service quality improvement, and perceived compatibility. In addition, factors significantly influencing physician’s continuous use including perceived usefulness, perceived risk, expectation on medical service quality improvement, subjective norms, and user satisfaction. Finally, implications for academics and practitioners are also provided.
