探討醫院評鑑之工作壓力對工作滿意度、工作態度、工作績效及組織承諾之影響

A Study of the Relationship among Accreditation Pressure, Job Satisfaction, Work Attitudes, Job Performance, and Organizational Commitment in Hospital Settings

摘要


我國自1977年辦理醫院評鑑, 1988年開始實施醫院評鑑暨教學醫院評鑑,將品質管理的 觀念與原則引進醫院;2007年起國家發展政策,為符合國際醫療外國人士來台就醫規格,引進 JCI評鑑制度,透過每三年一次的醫院評鑑活動,協助醫院發掘問題、指導、督促改善以符合 逐年提高之評鑑標準,也因此造就台灣醫療成就。自2012年以來,包括國家地理頻道、紐約時 報、時代雜誌及CNN等國際媒體專題介紹我國醫療成就,2013年The Richest更將我國評比為國 際第一名的醫療照護。 本研究主要目的是探討醫院評鑑帶給員工之工作壓力,對於員工的工作滿意度、工作態 度、工作績效與組織承諾等五個因素之影響,透過對醫院員工問卷調查,以印證醫院評鑑之工 作壓力與各因素彼此的相關程度與關連性,醫院管理單位應該協助員工改善醫院評鑑之工作壓 力,避免因壓力導致員工降低工作滿意度、工作態度、工作績效與組織承諾等。 本研究問卷的設計以Likert 五點量表,採用結構方程模式(Structural Equation Model,SEM) 探討醫院評鑑之工作壓力對工作滿意度、工作態度、工作績效與組織承諾之影響因果結果,研 究方法以LISREL分析軟體進行驗證。研究結果提供管理者對於準備評鑑工作時運用的建議, 以建構良好的工作環境,加強工作人員對壓力管理及調適的訓練。另外適當的工作壓力有助提 升工作績效,但過重時將適得其反,而造成人員滿意度降低、員工流動率與離職率增加,造成 組織花費更多的成本。

Taiwan conducted hospital accreditation from 1977. In 1988, hospital accreditation is conducted to teaching hospital. The concepts and principles of quality management are introduced to hospital. To comply with international health standard, attracting foreigners to come to Taiwan for medical treatment, the JCI evaluation system is introduced in 2007. Through triennial hospital accreditation activities, help hospitals to explore problems. Guide and supervise improvement to meet the evaluation standards increased year by year. Thus help to create Taiwan's medical achievements. Since 2012, international media including the National Geographic Channel, the New York Times, Time Magazine, and CNN have topics to introduce medical achievements of Taiwan. In 2013, the Richest rank the medical care of Taiwan as number one in the international competitions. Through the surveys of hospital staff, the structural equation modeling (SEM) and LISREL statistical software are used to investigate the relationships between work pressure, job satisfaction, work attitudes, job performance, and organizational commitment. Results reveal that work pressure will reduce job satisfaction, job performance will increase as work attitude improve, and job performance positively affect job satisfaction and organizational commitment. These findings is consistent to the opinions of most scholars. Based on the results, this study also provides recommendations as reference for the hospital operators.
