專科護理師客觀結構式臨床測驗教案的信效度

客觀結構式臨床測驗(Objective Structured Clinical Examination,OSCE)是目前評估專科護理師專業臨床能力方式之一，但目前仍缺乏OSCE考題發展信效度檢測，引發本研究動機。本研究探討臨床專家、學界專家評核專科護理師OSCE教案之效度及分析考官執行OSCE考題測試一致性的信度。以立意取樣台中市某區域教學醫院專科護理師共30位為研究對象，利用Microsoft Office Excel 2010 和SPSS17.0進行信效度分析。研究結果顯示胸痛教案專家效度指標0.96-1.0，評分者一致性Cronbach’s α為0.705-0.846。Kappa值為0.656，根據 Kappa值判定標準顯示評分考官判定結果的高度一致性；本研究結果佐證專科護理師OSCE教案考題具有良好的信效度，並提供未來做為檢測專科護理師在臨床上專業技能參考及建議，更進一步期許本研究的結果提供未來專科護理師教育者及實務工作者於發展OSCE評量之參考。

Objective Structured Clinical Examination (OSCE) is one of the methods to assess the clinical skills of nursing professionals, having reliability and validity test as part of the assessment process. The research focused on the reliability analysis of the OSCE assessment from both the theoretical and clinical experts； and the validity analysis of the test administrator’s consistency in the practice of OSCE. Thrity nurse practitioners from a regional teaching hospital in Taichung were chosen as the subjects, and the researcher utilized Microsoft Office Excel 2010 and SPSS17.0 as the tool to conduct reliability and validity analysis on chest pain.The validity scaling of expert was 0.96-1.0，The inter-rater reliability of Cronbach’s α was 0.705-0.846。The Kappa value was 0.656. The resulting Kappa value demonstrated high consistency in the test administers’assessment, and it also suggested the OSCE test questions may be one of the indicators to evaluate the clinical expertise of the nurse practitioner due to a positive feedback from reliability and validity test. The researchers hope that the result of this research may serve as a reference to the future nurse practitioners in regards to OSCE’s development.
