應用內視鏡的探測來移除上顎竇內之牙齒殘根－病例報告

摘要

異物掉入上顎竇內之情形並不少見。傳統上，要移除竇內之異物，大多是施行Caldwell-Luc手術，在口內上顎犬齒窩處做一切口，直接進入上顎竇內。本篇報告是介紹在全身麻醉下，將內視鏡導管放入左側鼻腔之下鼻道，穿過鼻壁後進入上顎竇。找到牙齒殘根後，將殘根從原來拔牙傷口處取出。口竇瘺管則利用腭中旋轉島狀皮瓣修補。利用內視鏡來移除上顎竇內之牙齒殘根，可減少手術傷口，也可避免開放性手術所造成的併發症，故提出此病例以供參考。

Foreign bodies in the maxillary sinus are not uncommon. The traditional approach for removing such foreign bodies is Caldwell-Luc procedure. This report described that removal of tooth root from maxillary sinus with an aid of endoscopy. The patient was admitted. Under general anesthesia, the endoscopy was put into maxillary sinus via nasal inferior meatus and removal of tooth root through the previous extracted socket. The oroantral fistula was repaired with rotational palatal flap. Using the endoscopic approach, it will decrease the wound and complications of traditional open surgery. Submitting this case for further discussion of diagnosis and treatment.
