端粒酶與增生性疣狀白斑的關連性

Correlation of Telomerase Activity and Oral Proliferative Verrucous Leukoplakia

摘要


在台灣,口腔增生性疣狀白斑的罹患率高於其他國家,轉變為口腔癌的機會亦高。由於端粒酶的活性在癌化過程中扮演重要角色,為了暸解口腔增生性疣狀白斑與口腔癌關係,本篇將研究增生性疣狀白斑及口腔腫瘤組織中端粒酶的活性變化。實驗檢體取自台中榮總口腔顎面外科之患者,32例為口腔增生性疣狀白斑,30例口腔鱗狀細胞癌,12例頸部淋巴腺轉移,所有取得檢體的患者都有完整的病歷記錄。檢體的端粒酶活性是以TRAP方法測量,並以吸光值(Optical Density)作為端粒酶活性表現值。結果發現口腔增生性疣狀白斑呈陽性比率為91%,其平均吸光值1.242高於口腔癌陽性率66%、平均吸光值1.020。晚期口腔癌端粒酶活性值有下降趨勢,且呈陰性反應者亦多,口腔癌頸部轉移淋巴組織的端粒酶活性也比原發部位口腔癌組織低。從研究結果發現,口腔增生性疣狀白斑端粒酶活性高,顯示其有很高之增生性,可視為口腔癌前期腫瘤。

Oral proliferative leukoplakia (OPL) is a highest incident tumor in Taiwan rather than in other places of the world. Most of this kind of tumor will transform to oral squamous cell carcinoma (OSCC) sooner of later. It is well documented that telomerase activity is related to carcinogenesis. In order to understand the mechanism of the OPL transforming to OSCC, the telomerase activity of the OPL and OSCC tissue are studied. In this study, 32 OPL, 30 OSCC, 12 neck metastatic lymph node tissue (from OSCC) were measured them telomerase activity. The patient’s clinnal parameters were well documented. The telomerase activity is measured with TRAP analysis, and O.D. value (Optical Density value) uses as telomerase activity level. The results show that the positive percentage and mean of O.D. value of telomerase activity in OPL tissue (91%, 1.242), which are higher than those of OSCC (66%, 1.020). And it shows that telomerase activity was decreased in later stage of OSCC patients. The cervical lymph node also showed lower telomerase activity than oral cancer tissue. In conclusion, OPL is a proliferative tumor with higher telomerase activity. It can be regarded as a precancerous lesion.
