居家醫療整合照護之中醫經驗與展望

摘要

臺灣的人口逐年老化，多重疾病以及失能的健康照護成為當前的重要議題，以病人為中心的跨領域居家醫療整合照護勢必成為一個重要的醫療照護潮流，中醫如何在現行的居家醫療的基礎上發展具有中醫特色的居家醫療是目前全中醫界所關注的問題，中西結合的居家醫療整合團隊必定能為病人帶來全方位的居家醫療照護。本文以一社區個案為例，分享失能患者的中醫居家醫療照護經驗，居家醫療整合團隊的中醫師提供中藥處方、傷科推拿、針灸治療；西醫師提供西藥處方；還有其他如心理師、物理治療師、藥師、護理師、社工師以及營養師等等各領域專業人員一起合作照護病人，文中並討論中醫介入居家醫療照護的優勢、挑戰與未來的展望。透過中、西醫師以及各領域專業醫療人員對病人健康狀況的全面了解，針對病人不同的身體不適，採取適切的治療方法。居家醫療團隊的照護內容包括對個案及其家屬的醫療照護、心理支持及友誼建立。總之，提供以病人為中心、跨專業合作、全面性、連續性的人性化醫療服務必將是居家醫療照護的發展方向。

With the increase of elderly population in Taiwan, the health care for people with multiple diseases and disability becomes an important issue. The patient-centered, interdisciplinary integrated home health care must be a consequential trend of medical care. Physicians of TCM begin to concern the development of home health care of traditional Chinese medicine (TCM) in the existing home health care team of western medicine. The integration of TCM and WM in home health care will bring more comprehensive care to the patients in community. In this article, we share the experience of home health care of TCM in integrated home health care team for a disable patient in community. The interdisciplinary collaborative home health care for the patient include Chinese herbal medicine, manipulation, acupuncture by physicians of TCM, medicine by physicians of WM, and other efforts from psychologists, physiotherapists, nurses, social workers, dietitians and so on. We also discuss the advantages, challenges and future prospects of TCM in integrated home health care. Physicians of TCM and WM provide different treatments for various discomforts of patient through a comprehensive understanding of her health conditions. The team of integrated home health care provided medical treatment, spiritual support, and companionship for patient and her family members. In sum, humanized medical service which provides patient-centered, interdisciplinary collaborative, comprehensive, persistent, and medical service for patients will be the direction of development of home health care.
