台灣急診主治醫師工作現況與合理工作環境之比較

Comparison of Actual and Ideal Working Status of Emergency Attending Physicians in Taiwan

摘要


目的:合理的工作環境是急診醫學發展重要的關鍵之一,本研究的目的,在了解目前從事急診工作的主治醫師之工作情形,並與他們所認為合理的工作狀況做比較及分析。 方法:本研究以問卷調查的方式,對象是全國地區醫院級以上醫院之急診主治醫師。回收的問卷中針對急診主治醫師目前的工作狀況以及他們所認為合理的工作狀況進行一致性分析,同時比較不同層級醫院之間的差異。 結果:本研究共收集到52家醫院,148份有效問卷。比較其工作現況及其所認為合理的工作狀況上,以排班方式的一致性最好(一致率為56%,Kappa值=0.345),其次是每月大夜班數(一致率為39%,Kappa值=0.115),一致性最差的分別為每月工作時數(一致率為16%,Kappa值=0.052)以及月薪(一致率為12%,Kappa值=0.032)。醫學中心在每月工時的一致性最低(一致率為9%),地區醫院則在假日班數上一致性較差(一致率為20%)。 結論:目前急診主治醫師工作的環境,與所期待合理的工作環境仍有一段距離,面對整個醫療環境的惡化,急診醫師應該更注意如何建立一個合理的工作環境。

Objectives. Ideal working status is an important factor in emergency medicine. The purpose of this study was to compare the actual working status and ideal working status of emergency attending physicians. Methods. Questionnaires were used to collect the data from emergency attending physicians in Taiwan. The contents of questions included physicians' actual working status and the working status which they was ideal. Results. A total of 148 questionnaires were collected from52 hospitals. Actual and ideal working status correlated well for working hours of duty (correlation rare = 56%, Kappa=0.345) and frequency of overnight duty (correlation rate=39%, Kappa=0.115), but did not correlate strongly for total working hours per month (correlation rate=16%, Kappa=0.052) and salary (correlation rate=12%, Kappa=0.032). Conclusions. There were large differences between actual and ideal working status in emergency departments in Taiwan. More attention needs to be paid to this issue to improve the working status of emergency physicians.
