急救責任醫院急診醫師工作滿意度之研究

摘要


動機與目的: 急診醫師除銜接病患「到院前」至「到院後」之醫療工作外,工作繁重,而國內對於急診醫師之工作滿意度卻少有學者加以研究,本研究期望國外之急診醫師工作滿意量表經修改後亦能合於國內使用,使急診醫師之工作滿意度能有效加以測量,並提供政府、醫院管理者了解急診醫師之需要,進而改善、提高其滿意度,而達到留住優秀人才、提高緊急醫療救護品質之目的。 研究方法: 本研究經詢問高雄縣、市及屏東縣地區教學以上急救責任醫院急診室人員該院之急診醫師人數後,以此為母體,自民國94年4月6日至5月10日止共發出149份問卷,回收102份,回收率為68.46%。在醫師個人變項方面分為性別、年齡、婚姻狀況、職稱、所得、有無急診專科醫師資格等。而工作構面分為行政自主、臨床自主、資源獲得、工作關係、生活型態、工作挑戰、整體滿意等7個構面。醫院型態以類別分為私立醫院、公立醫院,以層級可分為醫學中心、區域醫院、地區醫院。本研究依問卷調查所得資料,以SPSS 13.0統計軟體進行分析,以次數分配、信、效度檢定、因素分析、T檢定、單因子變異分析、LSD事後比較法、皮爾遜積差相關等統計方法進行資料處理分析。 研究結果: 一、急診醫師的工作滿意可透過7個構面來測量,並可分析出24個因素。 二、主治醫師在個人變項方面,婚姻在總滿意度上達到顯著的差異,收入、有無急診專科醫師資格、專職或兼職、不同類別、層級之急診醫師在總滿意度上無顯著的差異,年齡、急診室工作年資在總滿意度上呈現正相關,服務年資在總滿意度上呈現負相關。 三、住院醫師在個人變項方面,婚姻、收入、有無急診專科醫師資格在總滿意度上無顯著的差異、專職或兼職、不同類別之急診醫師在總滿意度上無顯著的差異,不同層級之急診醫師在總滿意度上達到顯著的差異,年齡、服務年資、急診室工作年資在總滿意度上呈現正相關。

Purpose of this research: Emergency physicians (EPs) are overloaded beside that they have to deal with medical management “before” and “after” patient arrive the hospital. Very few studies were conducted to analyze the satisfaction of the EP’s work. The Emergency Physician’s Job Satisfaction Scale in other countries were modified and applied in this study. This study hope to provide an insights for our government and hospital authorities to understand EP’s needs so as to increase their job satisfaction and improve the quality of our hospital services. Research Method: The samples took from emergency physicians working in local and regional teaching hospital and medical center in Kaohsiung county, Kaohsiung city, and Pingtong county. Between April 6th, 2005 and May 10th, 2005, 149 surveys were distributed and 102 surveys returned, with a return rate of 68.46 percent. Independent variables of the EPs are analyzed by their gender, age, marital status, work position, income, and with or without Emergency Physician Qualifications. Their working constructs are: administrative autonomy, clinical autonomy, resources, relationships with others, lifestyles, working challenges, and overall satisfaction. Types of hospitals are categorized by private and public hospitals, as well as by their ranks in Central hospitals, Regional hospitals, and Local hospitals. SPSS 13.0 was used to analyze the results of this survey with frequency analysis, validity and reliability check, factor analysis, T-test, one way ANOVA, LSD, and Pearson Square analysis. Results: 1. Emergency physicians’ job satisfaction can be measured through seven constructs and 24 factors. 2. Among the EP’s Visiting Staff’ independent variables, significant differences were found in the marital status, but no significance was found among their income level, with or without Emergency Physician Qualification, their age, nor their part- or full-time status. Significant differences were also found among the doctors in different ranks of hospitals and their age, years of wok were found to have a positive relationship with the overall job satisfaction. 3. No significant differences were found in the Resident Doctors’ marital status, income level, and with or without EP qualifications. There were also no significant differences in the EPs’ part- or full-time status, nor in public or private hospitals. Significant differences were found among EPs in different ranks of hospitals and, their age, years of service, and years of work in the emergency were found to have positive relationships in their overall job satisfaction.
