Post-juvenile Periodontitis Treated with Adjunctive Orthodontic and Prosthodontic Therapy: Report of a Case with Four Years Follow-up

合併牙周矯正及贗復治療後年輕型牙周炎四年追蹤：病例報告

摘要

傳統以來，我們一直忌諱在罹患牙周疾病的病人身上進行矯正治療，因為擔心矯正的力量會加速齒槽骨的吸收，再加上矯正器會造成清潔上的死角，所以牙周病患長久以來一直是矯正治療的一個禁忌，但是隨著牙周病學及矯正學的進步，牙周病治療的目標不再僅只是功能性、健康性，還必需考慮到整體的美觀性。本文是一位25歲女性後青少年型牙周病患者，主訴#46牙周膿腫及Class III malocclusion，在經過牙周手術治療後進行矯正治療及假牙膺復後，進而得到一個健康的牙周狀況、理想的咬合及美麗的外觀，同時在4年後的追蹤檢查一切情況良好，因此後青少年型牙周病患者在醫師謹慎的監督之下一樣可以進行矯正治療。

It has been long considered that a periodontitis patient is not a good candidate for orthodontic treatment. The orthodontic force may accelerate the destruction of alveolar bone and the orthodontic brackets will impose plaque accumulation, which will have adverse effects on periodontal health. However, for a periodontitis patient， a multidisciplinary approach involved periodontal and orthodontic treatment will be required to establish the ideal goal not only in oral health but also in function and esthetics. In this case report, the management of a 25-year-old post-juvenile periodontitis patient with angle class III malocc1usion through multidisciplinary approach was presented. After scaling, root planing and surgical therapy, the patient received orthodontic treatment with both fixed and removal types of appliance. Final prosthesis was then rehabilitated and the patient was under regular recall. A satisfied outcome was maintained even after 4 years.
