本研究目的在探討非中低收入戶失能老人使用居家服務及其相關因素,包括日常活動功能、滿意度、購買服務之意願及其預測因子。研究工具為自擬問卷及長期照顧管理中心使用的ADL量表,研究對象則為使用居家服務已滿一年的140位65歲以上之老年人。調查資料顯示被研究者每月使用居家服務1,176人次,合計3,268小時,比一年前增加了168人次(1,050小時)。每月使用居家服務的時數與老人是否獨居、家庭月收入、依賴程度、是否罹患關節炎、中風等因素有關。自費購買服務的預測因子包括年齡、教育程度、家庭月收入。整體而言,研究對象對於居家服務感到滿意,並肯定政府此項補助方案。本調查結果建議為有效推動照顧服務產業發展方案,政府除補助非中低收入戶失能老人使用居家服務,應於使用者有需求時,增強其購買的意願,宣導部份負擔的觀念。另由於民眾對居家服務人員之服務技術滿意程度最低,故應也著重居家服務人員教育訓練,以提升服務品質。本研究結果可供長期照護政策與居家式服務實務面參考。
The purpose of this study was to explore factors related to the utilization of home help services of the non-low income disabled elderly persons in Kaoshiung city, including patients' satisfaction and their willingness to purchase such services and the associated predicative factors. The subjects eligible for this study consisted of 140 elderly persons who were sixty-five years old and over and had been on the home help services for at least one year. The study used a constructed questionnaire and the subjects' ADL information provided by the Long-term Care Management Center. The results of the study showed a total of 3,268 home-service hours (1,176 visits) a month used by the subjects, an increase of 1,050 hours (168 visits) over a year ago. The number of monthly hours of usage was affected by the living condition of the participants, family monthly income, the ADL classification, and whether they were disabled due to a chronic condition such as arthritis or stroke. The multiple logistic regressions indicated that the purchase of home services was influenced by age, educational status, and family monthly income. Overall, participants felt satisfied with both home help services and the subsidy program provided by the government for long-term care. To increase the use of home help services and the elders' willingness to pay for the services when needed, the study suggests that the government should educate patients on the concept of balancing subsidy and payment by oneself for the home help services. Also, to improve users' satisfaction with the services, it is essential to provide in-service training for the service attendants. The results of the study may provide implications for healthcare policy and for practicality of home help service.
