摘要

長期且預後良好的贋復與修復牙科最基本的目的在維護牙周組織的長期健康，其對周圍牙周組織的影響，包括局部性或全口性牙周疾病，而這些疾病主要來自於牙菌斑的囤積與牙周組織變化有關的咬合形態與功能的改變。許多研究指出，成功的贋復與修復牙科能夠有效地恢復牙齒美觀、容易控制牙菌斑、發揮咀嚼的舒適性及減少測方食物嵌塞。相反地，不良的修復與贋復工作也會引起牙周組織的傷害並造成牙菌斑囤積的環境，長期地誘發牙周疾病的產生。因此這種贋復與修復牙科和牙周病考慮因素相輔相成、息息相關的共生關係，強調贋復與修復牙科和牙周病考慮因素之間相互影響的重要性。本文的目的在於提供贋復及修復牙科及相關牙周病考慮因素的理論根據及方針，以作為贋復與修復牙科重建美觀、健康、衛生、舒適的牙齒功能及牙周環境之依據。

The preservation of a healthy periodontal tissue is the most basic objective in long-term prognosis for the restorative and prosthodontic dentistry. The effect of restorative and pros-thodontic dentistry on the surrounding periodontal tissue, including localized and/or generalized periodontitis, which primarily resulted from retention of microbial dental plaque and alterations in the supporting structures, such as advancing periodontitis, the associated loss of teeth, lack of occlusal stability, and periodontal trauma from occlusion. Reports indicated that successful long-term restorative and prosthodontic dentistry will result in the most esthetic in appearance, access for plaque control, masticatory comfort and reduction of lateral food impaction. The converse is true and predicable that ill restored procedures will result in both permanent violation of periodontal tissues and habour for microbial dental plaque. Chronically, this will leads to the dissolution of connective tissue fibers, apical proliferation of the junc-tional epithelium, and pocket formation. A progressive periodontal disease process ensues. This symbiotic relationship between the restorative and prosthodontic dentistry and the clinical considerations in periodontal situations emphasized the importance of periodontal considerations for the successful prosthodontic dentistry as well as the successful restorative dentistry for the presserva-tion of periodontal tissues. The purpose of this paper is to provide a rationale and a guideline for the periodontal considerations in prosthetic and restorative dentistry in order to recreate esthetical appearrance , healthy periodontal tissues, functional occlusion, and comfortable mastication
